you sign the form by entering date where needed. That is, enter the date of the day you filled the form on the field next to the one that has the signature.

For d card, try either of zenith, first bank and gt bank.

I said to myself my nxt post will be my testimony but couldn't just ignore this prayer chain...I key into this prayer,our app shall turn out positive,it shall be so impossible for any V.O to say no to our request,favour is at work for us,all loopholes are being filled with his extravagant grace...we have our trv already,all that's left is songs of praise,dance and thanksgiving,

Well, that's not the main issue here; You said you started the business right after school so let's say u finished school 2010 ( the earliest) and since then have you worked in a field related to your course of study or IT related? Is your business IT related?

I noticed there was no NYSC cert among your listed docs, sorry to ask but were you above 30 when you served? If you might have been away from the IT field for about 4 years or more etc It might be harder to convince the v.o....

I included my nysc certificate , I just forget to list it here. Yea I av been away from IT field for about 4 years cox d business am doin is a family business not related to IT but I stated it in my SOP that I want to go back to IT field.
