Diclofenac ke? Someone with sepsis, and a possibility of DIC? Diclofenac in a patient with possible renal complication? ( usually the first organ to go in sepsis is the kidneys, and you don't wanna do any nsaid otherwise you worsen the renal status). Paracetamol is still the safest. You can't go wrong with it.
You want to stay away from nsaids. Paracetamol is still the safest antipyretic in my opinion.

I was told to give IV PCM 600mg stat. I then reopened the wound, washed with NS, and redressed with savlon and povidone iodine.

She was a Yoruba Christian. She was praying, holding my hands, saying that I should not let her die, that she still has little children at home. She then started behaving abnormally, talking irrationally and screaming, also convulsing intermittently.
At this point, I called a senior who came in, assessed her and told me privately that she won't make it. He told me to continue masterly activity which I did.

She finally died after few hours she was brought in. I certified her dead in her file, disconnected all life support, and went to my house to sleep around 3am when the call got less busy. I was weak and sad. Did you know, this woman followed me home. I was shocked when I felt there was someone looking at me from my window. My body was shaking. I've never believed in ghosts, but she came to my window and stood there, with same clothes she was wearing when she was brought in. She said why did I allow her to die. I was lost for words for a moment, thinking I was dreaming. But damn, it wasn't a dream. I just drank coke from the fridge few minutes ago. I rebuked her in Jesus' name, telling her I did my best. I told her to go back to where she came from. She stood there for a while, looking at me, then turned and walked away. I heard her footsteps as she walked away. I didn't sleep again till morning. I opened the fridge and took out coke and drank, to be sure I was not dreaming, then started praying in tongues to control the fear in my heart."

What can be a reasonable explanation for this?

If its real, you possibly did your best.

The ghost experience can be best considered as hallucination.... Try to get your mind off it and ignore such if it appears again.... If it persist, you may need to see a psychiatrist.....

Her spirit followed you. She was still hovering around but I thought she was a Christian. Familiar spirits can also take her from and do that. Was your house far away or close by?

Where were the senior doctors at that he had to call them on phone to seek for guardance..

I was once chased by a ghost too. I was way younger and she came from a naija movie I watched same day I saw her. She was so real...I still have leg injuries to show for it. It is well...may God keep us safe.

On a lighter note, op must be a fresh blood. Nothing wrong with informing your senior colleagues of a new case,and taking directives...... That was the right thing to do.
Choice of antibiotics too were the standard.
Just the anti pyretic that came too late.
Could have been one of the first 3 things to start with on her at presentation.

Lesson learnt, but u didn't kill her. Moreso, the 3days post op story is not likely to be true as we know patients and their relatives tell lots of lies just to save face and cover up the truth; common most times,and op will get to know this as he advances in practise.

To those calling for the doctor's head, stop it! Dnt comment on what u know nothing of.

In conclusion, the health sector in this country is begging for a complete restructuring.......things are not right. Most government owned hospitals in this country, including the supposedly big tertiary centres, are just mere consulting hospitals with little or no equipments. Empty buildings built by greedy politicians in a bid to siphon money, which are commissioned by people we look up to!

Well, u should have prayed for her after confirming from ur senior doctor that her case was medically hopeless. Mk16:18 states that will lay ur hands on the sick, and they shall heal. That would have been the game changer. Let's not rely only on our skills to save life, but on God thro Jesus. Try reach out to her family as someone rightly suggest. Remain blessed

Post traumatic stress disorder that's it op.Ptsd.Thats what Dr's have to go through and you have to shake it off and go to work the next day.

Keep doing the best you can,pray and brush it off.don't let your mind break,there's a lot to be depressed about.trick is face the next day better and stronger.and if you can't cope,please leave the system

It was in real life. I watched a movie about a ghost during the day. That night, it was my turn to lock up the small gate that lead to our staircase. It was kinda dark and the said ghost appeared after I had locked up. It didn't say anything, was just looking at me. I screamed and ran upstairs falling and standing all the way up...I had a good number injury on my legs by the time I reached upstairs. I thought I would die because it was the longest 1minute of my life...my heart was on fire. Whew! I know what I saw dude...it was real! As real as the boko insurgent.

I have read lots of funny comments here, am finding it difficult deciphering how some pple think sha. The Airforce1 nd carzola comments are rily hilarious, foolishly and ignorantly hilarious I mean. In big hospitals a whole unit of doctors will call on another unit to come review a patient, just like dis good brother did.it is called a consult and it is d bedrock of advance healthcare delivery. Notifying a senior colleague, especially in critical situations, makes effective medical delivery optimal. Even if yu lack common sense,it is beta not to show it all d time by passing judgement while totally ignorant of the topic being discussed.

this is why lay people like you for example should stay away from medical posts.Theres no where in the post the op said he gave an NSAID like dicoldenac.Any medical student anywhere in the world knows it's a no no for a patient with renal failure or anyone suceptible to renal failure

secondly, management of high grade fever is commenced by nurses before Dr's even see the patient,that's why he won't write the first steps in the write up cos the nurses would have handled that.

Summary,stay away from medical posts unless you have a degree that says medical

From what I am sensing in my spirit as I read this piece, I can assume that the woman knows the doctor spiritual capacity and that was why she said the doctor should not allow her to die. Also, the doctor said he prayed in tongues when he encountered the ghost, if truly you were baptised in the HolyGhost by evidence of speaking in the heavenly language, why not use that weapon when medicine was failing you. The HolyGhost was not just to aid you to speak in tongues only, He also teaches and show you what to do in this kind of situation. Lesson for another time doctor, a Christian doctor with the HolyGhost should truly be a solution sir

This was the turning point in this encounter. The young doctor practically lost the drive at this point. No sane Doctor gives a final verdict on a patient who is still alive.

PCM 600mg IV was the even the wrong choice of antipyretic for the situation , he was dealing with toxic overload and introducing another toxin like PCM was wrong , best choice should have been diclofenac with a potassium base
PCM is still the safest, Sepsis with possibly DIC
PCM is tolerable at therapeutic dose

have had countless encounters, infact my life is governed by the supernatural (which is christ Jesus)

There is more to this life than what we see, hear and feel

your life itself is supernatural, so the earlier you give your life to the creator, the betteer, give ur life to christ, my brother, if u havent done so

Everyday you keep getting bullied. Someone who suffered post surgical sepsis is already as good as dead? You lay men just assume doctors are miracle workers and who ever dies in their care shows incompetence on the part of the doctor. See how you have been used to mop floor online that you couldn't even reply anymore. Stick to your music that every Adams and James do which you are not succeeding at and let those making waves in their fields of study do their thing.
