This is sad.

I think the boy died while they were fiddling with the oxygen. That's probably why the Dr turned off the light.

The signs were there, and Tobi should have been taken away from that hospital since the 1st day.


Tobi is dead now and nothing can be done to bring him back. However, the Dr and hospital must pay in every way.

Tobi, my only child (a male, good-natured 14-year-old) was admitted into Duro Soleye Hospital, Allen Avenue, Lagos State on the morning of Saturday, 10 June 2017. He died on the morning of Tuesday, 13 June 2017 due to the negligence of Doctors Korede and Awe , who were on duty and their employers, Duro Soleye Hospital.

What happened:

1. When Tobi was admitted, the hospital, surprisingly, had no wheel chair to take him to his ward. The nurse on duty put him on my back to get to the ward.

2. Duro Soleye Hospital placed Tobi on oxygen on Sunday, 11 June 2017. The oxygen cylinder was plastered and obviously not in good condition (see attached pictures). This had me worried, but I was more concerned about my son's survival.

3. Tobi had a CT scan at Echoscan on Monday, 12 June 2017. Duro Soleye had no functional ambulance to take us to Echoscan for the test, we had to pay N5,000 for the laboratory to bring their ambulance to pick Tobi up.

4. I repeatedly had to call the attention of the nurse or doctor on duty to attend to Tobi, to replace the drip, or to carry out their lawful duties. The nurses on night duty were never prompt or responsive, always complaining and reluctant to do their work. Throughout our stay in the hospital, the bed sheets were never changed. The floor was also cleaned with only water, instead of disinfectant.

5. We got the CT scan results at about 5pm. The doctors [OE2] on duty saw the results and had no clue how to proceed with the treatments. This was clear evidence of the doctors manifesting clear incompetence in the assessment of Tobi. At that point, I began to panic but could not take him to another hospital because he was already on oxygen. The doctors on duty tried in vain to get directions from the Medical Director, Dr. Odunowo, but could not reach him till about 11pm.

6. In the evening of that same day (i.e., Monday, 12 June 2017), I asked the lady doctor on night duty (Dr Korede) and the nurses to change to another oxygen cylinder to avoid midnight exhaustion. I did this, because their practice was to sleep throughout the night. Regardless of my lawful directive as a paying client and mother of the patient, they refused to change it, insisting that there was still a little oxygen left.

7. The oxygen got exhausted while the doctors and nurses were sound asleep. The doctors and nurses had even forgotten to do the routine ward rounds and could not categorically say when the oxygen got exhausted! The doctors and nurses on duty failed to attend promptly to Tobi, even though he required urgent medical attention.

8. Due to my diligence as a mother, I discovered that the oxygen was exhausted, I called the doctor and nurses to change to another cylinder. None of them knew how to fix the new cylinder, they were arguing among themselves whether it was 'rubber type' or 'plier type'! They subsequently called an elderly man, hospital driver, from downstairs[ who fiddled and fiddled with it and seemingly "fixed" it. Prior to my discovery, the doctor and nurses failed to change the oxygen cylinder, a task which should have been done under any circumstance for the good of Tobi.

9. The doctor (Dr Korede), nurses and the elderly man then left the ward and the doctor switched off the light saying light can damage his eyes. This heightened my fear; after some minutes I switched on the light and called out to the doctor, who came and declared Tobi dead. It turned out that no oxygen passed from the cylinder to Tobi!

10. Also, mucus in his mouth was not sucked all through the night. Tobi was unlawfully deprived of his right to life by the actions and inactions of the medical personnel on duty. He died a horrific and painful death with plenty choking mucus (picture attached), which the Medical Director of the hospital, Dr. Odunowo, saw himself.

I informed the Medical Director of this development when he resumed for duty on Tuesday, 13 June 2017. I also wrote a formal letter (copy attached) to the hospital, which they refused to acknowledge. I gave the hospital my phone number.

I want justice for my son, whose life, dreams and aspirations were illegally cut short. One of his dreams was to study Medicine in the university, so he could help save lives. Unfortunately, the same institution he looked forward to being a part of, woefully failed him. Doctors owe their patients a duty of care and must never be negligent in their duties.

This story is incomplete in my opinion, it takes more than this to prove medical negligence. Firstly, what was the patient being treated for?! What treatment was given, and was it the right treatment? To prove negligence, the key criteria that MUST be met are;
Is there a duty of care?
Has the duty of care been breached?
Has the patient come to harm?
Sadly, the fact that this patient has died isn't enough to prove negligence in any court worldwide. We need the whole story, but we are just being provided with the story from the mouth of a grieving mother. In developed countries, a death like this automatically triggers a coroner's investigation on the cause of death. In Nigeria, we don't know the cause of death, so we blame the medical staff.

He did all the things you mentioned in your story and when he was informed that the baby needed oxygen after birth, and that the oxygen was low, he scoffed at the nurses and directed them not to disturb him.

He was actually am employee and was subsequently sacked from the hospital.

That young man insisted that my baby's head which had come out must be pushed back cuz he must operate on my wife.

Despite the SOS from the nurses the idiot left my dying wife in anger.

This same murderer instructed my wife to close her legs in that condition which would suffocate my child whom I'd already given the name Ellen Chigozie.

I ran wild with drinking for months...

I still feel pain.

I'm still bitter and I'm still planning to take my pound.

That child was my first blood.

I buried that child with my physical hands.

Like someone said earlier, you won't understand what people experience when we have to rely on our medical system if you haven't been there. My dad died from the negligence of the nearby hospital we rushed him to when he took ill. The doctor and nurse on duty slept till morning and then came to his room to check on him and pronounce him dead. A private hospital. His body was taken to the teaching hospital for embalment. When his body was being taken home two weeks later we found the right hand was decaying!
The consultants who run private clinics rely on junior doctors day to day. I remember when the junior doctor who saw my blood test report after I gave birth insisted I must take 2 litres of blood. I was the one who complained of weakness which made him do the test. I refused because I didn't want HIV.
He persisted till the consultant arrived in the evening and said I could take a drug he prescribed.
Less than a year later the news broke of a baby who was transfused with unscreened blood from the same hospital lab that supplied the blood I was to have been given. The baby became HIV positive. There are more instances but I'll stop here
Also pray that the emphasis is not on money. Some institutions are set up to make money. Rendering service for them is history so they'll string you along to get the most out of you.

My heart bleeds! There's no need for blame games. It won't bring the boy back but that death trap of a hospital should be shut down and all staff on duty arrested. They should be dealt with severely so that others would learn from it and be on their toes. To lose a child among ten kids is so painful not to talk of an only child. May his soul rest in peace.

to my dear commentators; please let us be careful how and what we post relating to commenting as we all can read and see that we are talking about a situation of a son who died here.

in light of that, let me quickly attend to those who are in the view of she moving the child out of the hospital once she sees that their services are not ok. it is my prayer first that you would not be in the situation describe above. you are talking clearly because you are not involved. just like it is very easy to pass comment in a football match as a commentator but put yourself in the same shoe and you are doing the same even if not worst.

let not allow our emotion to becloud our judgment there. if all our hospitals were functional and good to go, one should simply walk in to any hospital and the only factor that determines whether you stay or not should not even be the fee but the patient's choice.

our government have milked this country to the bone and that is why they can fly out of the country for just common ear problem.

it is a pity how many promising future have been killed by the heartless action of some hospital doctors who value money more than life.

I say UCH collectively which is a combination of hospital building, medical instruments and repair/calibration, direct primary medical staff like Doctors and secondary medical staff like nurses.

Each is an important leg of the stool that is absolutely required for a hospital to work effectively and efficiently. Remove one and proper patient care is missing and the result could be deadly. (Just look at the sham in the picture by OP).

We'll have to define what is considered best. Somalia I think is better than South Sudan but neither is as good as Holland or Italy, an analogy.

Two trips ago I had a crown that came off and I went to some dental facility near Cocoa House to see about affixing it back. One look at the place, the instruments and the total unhygienic environment I left immediately and endured a loose crown (very painful) for two weeks until I got back stateside. I won't even let my Dog be treated there. Are there better dental clinics in Ibadan? Possibly, but this one shouldn't be in existence.

Much of my career in the US has been spent in the hallways of most of the top rated hospitals in the developed world, so I'm a bit jaded.

Hmmmm.We're faaarrrr from having standard hospitals in this environment. For someone like you who has lived in the developed world,I can imagine the exasperation.
To be honest, most hospitals here operate at less than 50% of what the standard should be.

Duro Soleye is a very reputable and old hospital with a moderately high client flow. If they were so terrible, they wouldn't have such patronage. I cannot tell what your child was suffering from but I can say there was some mismanagement if your story is accurate. The oxygen tank is normally taped like that to prevent any leakages from the connector as it often wears out with age but that does not prevent it from working.
The doctors, however, failed to
1. fully enlighten you about your son's diagnosis and inform you about his progress or deterioration constantly.
2. the consultant(s) in charge of your case were not reachable. Knowing how critical he was, they should have been on standby.
3. those doctors on call do not have any excuse, tired or not, to refuse to attend to anyone as long as they have not handed over. We are trained to do up to 72hr shifts if necessary.
4. The nurses are not to go to sleep on night shift. They may doze at their desks but never retire. They are also to chart the infusions and oxygen, monitoring when they are about to finish so they can change them.

At least, for the time I was in private practice, this was the standard. I know there can be no pain worse than losing a child, how much more the only son you have. God is still on the throne and He will surely comfort you.

Be consoled. Please sue them. This is the third bad story I have heard about that Duro hospital. Take them to court and let's hear them talk.

It's sad to lose a loved one but i share the sentiments of someone that noticed that the woman was more interested in irrelevant details like the floor being mopped with water. What was your child being treated for? From the info, it was a severe brain condition, probably a meningitis. What were you told about it? Where you coming from another hospital? I hate ons sided accounts, let them do an autopsy already and please... i hope the findings will be pasted here too.

The doctor had poor communication skills and was obviously rude despite your dismay, he erred in that aspect, it is clear you never understood what went wrong and he made little effort to explain and counsel.

But why is the complainant hiding the fact that the said boy came into the hospital in deep coma with advanced stage meningitis?
why did she avoid answering the question about what was wrong with her son?

such a pain. but did you try another alternative since you weren't comfortable with their treatment or request him being referred too a better or more competent hospital. its like you knew there weren't competent but was using your kids life too take such risk.

I am sorry but I blame this woman...she acted like a woman without sense...if she already knew they were incompetent,why did she keep her child there...in my own opinipn,she killed that child....Albert Einstein said 'madness is trying the same thing repeatedly and expecting different results'..As a matter of fact,whenever I am in the hospital or clinic,nurses there don't like seeing me cos I will constantly be on their neck to do their job,i have heard serious altercations with medical practitioners when I find them acting unprofessionally....if you pay good money for healthcare,then by all means,you should get the medical attention...a few years ago,a particular nurse saw my mom who we brought in like an emergency because she was struggling to breath and simply looked up at us and said,tell her to sit down....what followed can better be described as a war of words...I apologised to her when it was all over and my mom was much better but something tells me she has nt forgiven me,but honestly,i don't care...

Sleep on little one...forgive your mom...she felt she was doing what was best for you...

Here is Her Account of what actually transpired:

Tobi, my only child (a male, good-natured 14-year-old) was admitted into Duro Soleye Hospital, Allen Avenue, Lagos State on the morning of Saturday, 10 June 2017. He died on the morning of Tuesday, 13 June 2017 due to the negligence of Doctors Korede and Awe , who were on duty and their employers, Duro Soleye Hospital.

What happened:

1. When Tobi was admitted, the hospital, surprisingly, had no wheel chair to take him to his ward. The nurse on duty put him on my back to get to the ward.

2. Duro Soleye Hospital placed Tobi on oxygen on Sunday, 11 June 2017. The oxygen cylinder was plastered and obviously not in good condition (see attached pictures). This had me worried, but I was more concerned about my son's survival.

3. Tobi had a CT scan at Echoscan on Monday, 12 June 2017. Duro Soleye had no functional ambulance to take us to Echoscan for the test, we had to pay N5,000 for the laboratory to bring their ambulance to pick Tobi up.

4. I repeatedly had to call the attention of the nurse or doctor on duty to attend to Tobi, to replace the drip, or to carry out their lawful duties. The nurses on night duty were never prompt or responsive, always complaining and reluctant to do their work. Throughout our stay in the hospital, the bed sheets were never changed. The floor was also cleaned with only water, instead of disinfectant.

5. We got the CT scan results at about 5pm. The doctors [OE2] on duty saw the results and had no clue how to proceed with the treatments. This was clear evidence of the doctors manifesting clear incompetence in the assessment of Tobi. At that point, I began to panic but could not take him to another hospital because he was already on oxygen. The doctors on duty tried in vain to get directions from the Medical Director, Dr. Odunowo, but could not reach him till about 11pm.

6. In the evening of that same day (i.e., Monday, 12 June 2017), I asked the lady doctor on night duty (Dr Korede) and the nurses to change to another oxygen cylinder to avoid midnight exhaustion. I did this, because their practice was to sleep throughout the night. Regardless of my lawful directive as a paying client and mother of the patient, they refused to change it, insisting that there was still a little oxygen left.

7. The oxygen got exhausted while the doctors and nurses were sound asleep. The doctors and nurses had even forgotten to do the routine ward rounds and could not categorically say when the oxygen got exhausted! The doctors and nurses on duty failed to attend promptly to Tobi, even though he required urgent medical attention.

8. Due to my diligence as a mother, I discovered that the oxygen was exhausted, I called the doctor and nurses to change to another cylinder. None of them knew how to fix the new cylinder, they were arguing among themselves whether it was 'rubber type' or 'plier type'! They subsequently called an elderly man, hospital driver, from downstairs[ who fiddled and fiddled with it and seemingly "fixed" it. Prior to my discovery, the doctor and nurses failed to change the oxygen cylinder, a task which should have been done under any circumstance for the good of Tobi.

9. The doctor (Dr Korede), nurses and the elderly man then left the ward and the doctor switched off the light saying light can damage his eyes. This heightened my fear; after some minutes I switched on the light and called out to the doctor, who came and declared Tobi dead. It turned out that no oxygen passed from the cylinder to Tobi!

10. Also, mucus in his mouth was not sucked all through the night. Tobi was unlawfully deprived of his right to life by the actions and inactions of the medical personnel on duty. He died a horrific and painful death with plenty choking mucus (picture attached), which the Medical Director of the hospital, Dr. Odunowo, saw himself.

I informed the Medical Director of this development when he resumed for duty on Tuesday, 13 June 2017. I also wrote a formal letter (copy attached) to the hospital, which they refused to acknowledge. I gave the hospital my phone number.

I want justice for my son, whose life, dreams and aspirations were illegally cut short. One of his dreams was to study Medicine in the university, so he could help save lives. Unfortunately, the same institution he looked forward to being a part of, woefully failed him. Doctors owe their patients a duty of care and must never be negligent in their duties.

Waoh!!! This hospital!!! This is really pathetic. I know the hospital very well but i will reserve my comments.

The mum should have taken the boy to another hospital immediately she noticed the first lapse. Who knows, he might have survived it.

May his soul rest in peace and may God grant His family the fortitude to bear the loss.

I am shocked Duro Soleye has lost their standard and would even have an apparatus like the plastered oxygen cylinder having known that hospital since I was a teen till my adult life. It is really shocking. Despite how expensive they are and yet this kind of dishearten story on negligence.
