I know the clear difference between the two. Gates was talking of vaccine. But drugs also pass through clinical trials as well. There are drugs/ treatment procedures that can even cause more harm than vaccines if not well tested.

An example was the meningitis drug tested on 200 children in Kano by Pfizer in the 90's. 5 fatalities were recorded. 7 others got lifelong effects.
