You need to swear an affidavit. Take the affidavit to NPC state headquarters in Surulere. You will get it after 2 days...It costs like 5k (Cannot remember exactly how much I paid)
I dont know the office address but it just next to NYSC state headquarters on Bab Animashaun street.

I have a similar situation, My father used pen to cancel the first 3 letter of my name on my BC. I did another BC from NPC.
I advise you do the same....to be on the safe side.

Have you submited ?
If yes ,no issues ...seen some buddies fly through with declaration of age.
If you are yet to submit ,you may strengthen your case by including an NPC attested birth declaration certificate.You may also include your first school leaving certificate which normally contains parents name and birth dates.

Just came from Lunch now and saw the pick up mail sitting DEEP in my SPAM box....
Its official......
Pick up mail arrived in my SPAM box at exactly 12.45pm.
I'm using this as a point of contact to expectant hearts of Medical request mails,passport request mails and Passport pick-up mails or any forms of positive news from Accra.
