You don't need the online form, just go there early with your biodata n online printout of receipt, you'll write ur name down n you'll be given forms to fill. Fill it, go for weight n height measurement, go for lab test n then book an appointment in d xray office. That's all. But take note of this, for lab test, make sure u get there b4 8am cos of d queue, they only attend to the first 40 plp. Have done mine except lab test cos I keep getting there late. Hope I helped

kk! I paid on Friday, had d same issue of not being able to print my reciept, went to d bank, complained n she printed it from her system!..I logged in now n am seeing d receipt option , which I wasn't before...
although whenever I click on medical form its still d same " pay ur fee stuff" I get to see.

those dat ain't able to access their medical form, even after paying. is it as a result of not being fully cleared or wat?....

academic building or somtin. its d first or second major building on ur left wen u enter tru d college gate. it has a huge desk ( reception) at d entrance. wen u get there just ask, u wud b directed. I think d name of d lady is Mrs Muhammed...n pls go wif ur #2000

I have submitted my form at my faculty office at luth, 2 days after my screening at Unilag.
now I hear som pepz shouting departmental faculty registration is next wk n am wondering if it is d same i have done!!!.......basic medical students pls help on diz one!

ayozainy: 1. Print out ur receipt from d students portal
2. Visit ur faculty/department 4 registration
3. Go 4 ur library registration, 4 jst N200
4. Fill ur medical form in d student's portal
5. Print out ur biodata and student information form
