Yh,like u've been seeing on this forum, we are to download it,den transfer it to Microsoft Word. (adedayo516 &segunflames,kindly explain d computer process here). Thats where he would fill it before printing it out. He should retry it,its gonna work or ask d cafè personnel 4 assistance. ....I guess its best he does it this way,coz they will surely tell him to do it dis way,after quein up 4long! To avoid stress.

As you've said he just should ask d cafe ppl for assistance.

Do not write but type is written under the acceptance form. Type/edit your details only on the acceptance form b4 printing. The form is in MS word format
