Well, the ones I know ask for residential address verification which could come as utility bill or bank statements with your name and the address that matches the ones you used while signing up.. Its a form of regulation for them, so they can't skip it.

We all know that when you request for an ATM cards, it takes about 2 or 3 weeks for it to get to you. And from the story, the guy died on the 10th and the withdrawal occurred on the 18th. OK. Its possible that he requested for a new card b4 his death, but a banker got to know about his death and decided to make use of the card as soon as it arrived. Remember that these bankers are the ones that hand over the envelop with your card and pin to you.

Video format is no longer an issue nowadays. If you watch your TV shows on PC, use VLC - it supports a large variety of video formats. It's Android direct equivalent is MX Player. If you don't have one already, then download and install one from Google playstore or Amazon Appstore.

Just peruse the interwebs with the help of Google, surely you will find what you seek. A question though, why must it be MP4? If you have vlc player you can watch it in any format, even the mkv format that seems to be everywhere.
