Seems the fellow involved finally divulged further info: he walked in and requested for Ciprofloxacin and was educated and offered azithromycin. A pharmacist is the custodian of drugs and their appropriate use. Pharmacists also by their charge educate patients and promote responsible self medication, health promotion and rational drug use. However, the pharmacist shouldn't have condoned irrational use of an antibiotic given the current drive for antibiotic stewardship to reduce antimicrobial resistance. A cursory look through appropriate clinical resource (central library of medicine- pubmed) however indicates that in asthmatic patients, Azithromycin use leads to lower primary end points i.e. reduces symptoms and exacebations. The best course of action would have been to discuss this with @Demilader's physician and seek an agreement to pursue this course. Barring an idiosyncratic reaction, the drug should actually have made @Demilader feel better. Pharmacists are able to provide generic substitutions for patients without recourse to the prescriber but are always required to discuss therapeutic substitutions with the physician (in practice settings where prescriptive authority exists for pharmacist prescribers, a short term intervention can be taken without consulting the physician but must be documented for review within a stipulated time). I do hope @Demilader lives well and heartily, and wish we would all just look at the facts and make progress. This incident should ordinarily have been documented and a proper clinical and regulatory review done.
