copy... Leave d photocopy own blank... Wen u get 2 schl u'll
know what 2 do and den u wouldn't lose in any way... 4 d
one's u have 2 affix passport... Print one make 4 photocopies
and put ur passport on all 4 ... In all u do make sure thr is
always an Extra Form in case of mistakes... Other stalites can
ammend or add 2 my point though... Happy Resumption <---- Remember This!!!
