Your question is like asking if one will still need to make legal something that is already legal by law! A typical affidavit (sworn) is issued by a court of law while a marriage certificate, as long as it is bearing Nigeria's coat of arms, on Form E (see a variant attached below) and duly signed are already legal and valid documents.

1. Use your current surname as you will need to verify your identity. Submit proof of change of name along with your SVR application
2. No need to notarize your certificate but you will need to notarize a copy of your passport and your certified identity document. You will get instructions for this when you request SVR
3. For the meantime due to COVID they are accepting soft copies but they said they will require the hard copies to be sent sometime in the future
4. I don't remember needing to specify this or giving a particular contact, they check it against the world directory of medical schools so maybe check there. I would assume it's the medical school that will be listed not the main campus

Oh I see, I'm glad you were finally able to open your physiciansapply account. Still try your naira card when paying for SVR and ECA because I just paid for my ECA a few days ago and it still worked again despite saying it needs to be a credit card. No I didn't notarize my degree certificate, only my identity document form and passport copy as per their instructions and it was accepted.

I paid for SVR yesterday and my card still didn't work though I'm beginning to think � its my bank. What bank card do you use.
I intend to get my documents notarized and sent before the week ends.

Hi.
I finally had to get in touch with MCC and they insisted it had to be a credit card, though I've finally crossed that hurdle.
Pls did you notarize your degree certificate?

Thanks for your response. I'm actually at the point of creating a physician apply account and I've tried using both a naira/dollar debit card but it keeps returning the message- Payment failed.

If you are a salary earner in Nigeria with an income above N250k or N350k, you can request for a credit card from your bank. Almost all banks in Nigeria now issue credit card

Hello house,
Pls i need help from any doctor in the house who has been able to pay for MCC SVR and ECA recently, it says payment must be from a credit card only! Seeing as credit cards are not so common around here, i need help on how to go about the payment.
