*1. University of benin student identity card
*2. Original and a photocopy Of statement of result
*3. Original and photocopy of screening certificate(clearance certificate)
*4. One passport photograph
*5. Registration proficiency certificates,evidence of registration with professional bodies for medical,optometry,dental nd pharmacy ppz.

1. Photocopy of statement of result
2. Original clearance form
3. Identity slip in lieu of id card.

Hopefully d list will be pasted tday but call up letters will be picked tmao definitely.
