It is not likely he acted on account of his status (be it fetish or malicious). That state is known to have a high prevalence of HIV anyway. Thankfully, it doesn't. . .and here's a shocking statistic: Its actually less than 1%!!. However, a lot of factors could affect that number, and include presence of infections (other STIs), sores on private area, viral load of the individual, and whether he/she is on ARVs, number of encounters, risk status, etc, etc.

The man is said to be a doctor and is currently on ARVs. That's a good sign. There is therefore the possibility that the viral level in his body will be low and this reduces the chances of him passing on the virus to the unfortunate victim. And like someone said, male to female transmission is slightly lower than the other way round.

I would say the odds are pretty low from this single encounter.
