Go ahead and eat the 'Bole', nothing wrong.Do you know that you need minute dose of bacteria sometimes to boost your immune system? It's the same way a vaccine works.
Not every time every thing has to be 100% clean. You need your immune system to be able to record these so called bacteria as bad, when in minute dose, so that you can have a strong defence when there's a major attack of the same bacteria to your body. It's like the american CIA gathering Intel to use for future purpose.
Oya medical personnels, make una help me out here.

@OP

The correct spelling is "BOOLI" pronounced as "boli" (Yoruba)
"Bole" as u wrote would have to be pronounced as "bohlay" if u said that beside me, I wouldn't know what u are talking about.

Now to the main point u raised concerning hygiene, well u are right that the environment in which it is roasted is unclean but it is important to note that bacteria can not survive under the condition where the Booli is being processed. 1) High temperature 2) Low moisture. Although the moisture content of the Booli increases on th inner part but since ur concern is on the outer part, there is not much problem cos the outer part is heated till its dry.

However, this does not mean that it is completely safe because of some die hard bacteria that may survive heat above 100 degrees but since it is heated, the risk probability is not as high as u might be thinking.

Here in the south-south, we eat bole and fish just as much as the Hausas drink fura de nono.Are they germ free,no, but they are ideal food,so long as you can keep the level of contamination minimal.Just as much as this is a delicacy we can't do without, I advice that you buy from people who ensure that they maintain the highest possible hygienic standard.I eat this thing a lot o but do check the seller and the environment before buying. I have suffered from strep throat twice due to bad judgement but I don't mind that little inconvenience in my throat, so long as my taste buds are excited. ;DHere in the south-south, we eat bole and fish just as much as the Hausas drink fura de nono.Are they germ free,no, but they are ideal food,so long as you can keep the level of contamination minimal.Just as much as this is a delicacy we can't do without, I advice that you buy from people who ensure that they maintain the highest possible hygienic standard.I eat this thing a lot o but do check the seller and the environment before buying. U dont like booli or you just want to give it a bad name? I lOve my booli I no go lie. Dusty or not, as long as it is red hot, every oda tin bros na wash!
