A good doctor is not one who plays undue heroics, but one who knows when to refer. Rather than all these recharge card buying and panic calling, your triage skills would come in handy, and whoever needs an IV access gets one, ditto other things. From me to you, don't ever mistake the social media for your clinical review or m & m meetings. Posting here has not achieved anything, rather than bringing the profession, and your personal practice to disrepute. Like I said earlier, pls don't ever repeat the goof, ever again. You'd toughen-up on the job.

Some time I weep with the way things are in this country, silly policies affecting negatively on the life of the poor while the rich jet around to any part of the world just for having stomach upset, I feel like doing a Mohamed Bouazizi just because of this silly policies made by this our leaders but we are too clouded by religious and ethnic sentiment....are the Tunisians who started the arab spring created by a different God or by the same God that gave all of us brains and boldness to fix things..

Sorry, he did not try his best at all. What would it have cost to defy that order from the senior Dr? After all this was a state hospital! How much would it cost just to perform some basic operations that might have kept that young lady alive?

Nigeria is a follow follow society. Even our best and brightest will follow "orders" good or bad. Hence our sad situation.

I understand (though I don't agree) with the young doctor's predicament. Money can always be gotten not life.

The country Nigeria is probably already lost. Hope we don't disolve into a state of anarchy like Somalia.

The fact that we deal with human life makes everything so sentimental so people will call you all sorts of names and ask you to lose your job. I have seen where the doctor is ready to work, but the nurses refuse to bring out the instruments because they are on strike. You can't disobey your senior. It is just like a military service system.

Besides, can you guarantee that you would have been able to save the woman's life? No law stops you from giving first-aid but can you continue to give the required care? You were not negligent in your duty.


It is a pity that we live in a country with a messed-up health system and you have to pay out of pocket for the most basic kind of healthcare. There is no social fund for accident victims in hospitals so anybody who is mad at you can go down to the nearest hospital and make a donation.

Many of the people criticizing you for not spending your money have never donated a single vial of normal saline (150 naira) or iv cannula (200 naira) to any government hospital. Just continue to do your best within your limits. Nigeria will be better soon

PLEASE WHAT CAN I DONATE TO AN HOSPITAL ? I JUST WANT TO DO MY PART IN MY OWN LITTLE WAY.

simple IV fluids and oxygen would have stabilised her b4 calling ur senior bt u didnt do that. Besides its not the job of a medical officer to collect money from patients in a govt teaching hospital so this your story is suspicious. OP I understand you are a HO but u failed to do ur job which is to stabilize the patient instead of heaping all the blame on ur senior. Its a lesson. Next time u resuscitate and do group and crossmatch and ur patient will be safe

Sad. Such can linger on your conscience for ever .. and ever. Take this as a learning experience.... this is the first time....and I'm almost sure would not be the last. I understand your position... and the need to follow instructions from above. I strongly advise you do as I do .... when in phone contact with your boss.... and the instruction is bad... or as in this case heartless and unethical...immediately say "you're breaking up please repeat what you were saying"... and before she can start repeating start saying " hello? hello?hello?...are you there?...hello?" then mumble some bad words about Mtn or whoever your carrier is and hang up...switch your fone off and promptly go about making your own decisions. If your boss is dumb enough to confront you on dropped call.....ask the idiot what were you supposed to do?

Immediate referral is a huge way of handling emergencies.
Based on your descriptions, the police should have immediately taken all patients to FMC where they have greater chances of survival.

Please I do not mean to blame you but I will say the following in case of the future:

In such emergency situation, physically going out to buy recharge card or call other doctors who MAY NOT be on duty at the time will only waste the little time such patients have to survive. Resuscitation protocols says you should call for help WHILE ATTENDING TO THE PATIENT.

It is unfortunate but based on your details, your centre did not have enough personnel or logistics to handle the situation at that moment.
The police need more enlightenment concerning the proper/best hospital to refer certain categories of trauma patients.

Another issue is the manner our health system is set up. The issue of payment keeps rearing its ugly head( although in this case, the patients were better of referred to a higher centre).

We need every citizen to be covered by the National Health Insurance Scheme.
If every citizen of this country had a health insurance as matter of policy, the issue of paying before treatment in emergency cases will cease to exist even in private hospitals.

In addition, I want to call on the NMA to work towards ensuring that House Officers will no longer be "First on Call" at our emergency units.
No doubt, a senior doctor would have taken better decisions in this case.

You are not a doctor, you have never worked in the Nigerian system, so just keep quiet. Even the foolish doctor that started this thread, knows that he is a major factor the patient died, not the hospital or any government policy. His ineptitude, incompetence, and half-baked knowledge killed the patient.

He was given a direct order by a senior doctor not to treat. In an ideal setting, he will answer a query. From a doctor's standpoint, if he went ahead to treat, he will not be punished.
But Nigeria is not an ideal setting. Doctors have been sacked for disobeying senior doctors. As a junior doctor he must be under supervision, a case can be made from there.
spare me this rhetoric. How will he be given query or sacked if he did simple resuscitation? The OP 's incompetence is very glaring in this situation. I will advise him to be more serious with his house job training because its just one year and its the basis of his future practice. The time he wasted buying credit and making calls would have saved the lives of most of the victims. Its a lesson that should not repeat its self again

An house officer can never be sacked. I repeat they can never be sacked!!!!. The worst that will befall them is extension of their postings or some extra calls. They are learning with a temporal licence so they can't be punished by law

Nonesense! The MO would be sacked not the other way round! This is a clear case of incompetence by both the HO and the MO. The MO obviously not wanting to be disturbed gives a foolish excuse! Since the HO prooved his incompetence by calling the MO first, the MO would have been on the phone instructing the the HO on steps to stabilize the patients.

It was around 11am today, 26th Dec 2015, I wz in the emergency room as the only Dr & House officer on call in a state Specialist hospital wen a peugeot wagon followed by a police van drove in. They opened d doors & trunk and started bringing out Accident victims. I did a quick triage & saw dat d 2 in d trunk were in a more critical condition, So d immediate plan was to refer d 2 to FMC wia dey hav better hands and equipment to handle it. So I asked d policeman who d driver is so dat d 2 will be rushed to FMC & he said he died at d Accident scene. I ran outside to buy airtime to start calling other HOs, b4 I could return, dey had dropped d 2 & drove off.
I called other HOs, only 2 showed up, others had travelled. To my greatest suprise, I called a medical officer who is a snr colleague in d proffession, she said I shud nt touch anyone till dey paid. That I shud Infact refer all of dem. I was incapacitated, d patients were in pains, crying, calling on us to help, some already unconscious, but we couldnt help cos we have an order frm above.
Within 5mins, 1 young lady in her early 20s died. Abt 10mins later, another police van came and carried d remaining 7 to FMC.
This evening, some road safety corps came to inquire if any Accident victims were brought to our center. They had retrieved d manifest frm d vehicle and contacted d families of d passengers. Dey came wit d relations of one of d patients, and wen I told dem 1 of dem died, dey asked for d Description of d 1 dat died and I showed dem d pix of d gal dat died, behold she was d 1 dey were looking for and right there was the gal's fiancee. Their traditional wedding was to be in 3days tym
I want to use this medium to call on the govt to make policies dat"ll enable Drs treat patients in such condition. Let's Save the human race

Cramjones pulled the UK story to show uninformed people here how serious this case could have been should the nigerian system worked properly, the OP would have been in jail. My advise to the OP is he should start taking his house job training very very seriously cause this is a warning sign to him for future lapses

In fact any field cos of shortsighted policies.our bodies are so unique n so complicated n you just finished 7 years learning just how to fix it..n you passed an opportunity to use that knowledge.keep getting tough...keep becoming heartless like your colleagues...I know your situation couldn't be helped in the logical sense buh deep down you know you could've done more

Broda pls stop being so angry. We understand the situation here: u got orders from above, saying "don't touch until they pay, blah, blah...but one thing u shld know is that sometimes, a little courage is all it takes to do the right thing and not compromise ur values.
Just like someone pointed out, if ur sibling was rushed in like that, I don't think u would have stood there and watch him/her die without doing at least one thing to help.
But I also understand that being the only medical personal on call at the time, u must have been feeling overwhelmed to do anything.
Fact is, it happens. So u might wanna learn to live with it. But I strongly advise that next time you face any critical situation such as this, just do what u believe to be the right thing, orders from superior or not.

It's not a must but sometime you can decide to stand in for such patients and resuscitate but with the resolution that you pay with your money if they dont get to pay up later. However, majority if not all gratefully pay or contact people who pay the bill when they become conscious. I have done that many times and always happy at the end. I am not saying that you have done any wrong by following order professionally but since you would be this troubled Maybe next time you should try and assist.

U guys must really be so ignorant. Yes it is a govt. Hospital. But it don't mean the services are FREE!
U still get to pay even before any doctor sees u to hear ur complaint.
Just that in this case where an emergency situation was involved, they would have been primarily concerned with keeping the victims alive before demanding for payment.
Nigeria is messed up as it is. Money currently is more valuable than human lives.

Nigeria still has a long way to go. See how we are joking with peoples' life because of money. I am a vet doctor and I hardly watch an animal in critical condition (especially cases of poisoning in dogs) die because of money or order from above even when I was working in a vet teaching hospital. Most times the reward you get after taking the risk of neglecting such orders and attending to patients in critical conditions worth more than gold. Sometimes we should be able to work with our conscience and take the risk just to save life. May God help us

The people bashing the OP deserve to be flogged! Some even brought up suggestion that the person pay out of pocket and be reimbursed later, what a stupid suggestion! I've yet to see one single person actually acknowledge the fact that money matters more than lives in Nigeria. Till we come to this drastic realization we are all just kidding ourselves! Where did it all go wrong? Politicians! They are the ones responsible for enabling laws which protects accident victims and those in need of urgent care. There should be a budget set aside every year for such expenses but rather they would rather spend money on luncheons and private dinners, private jets and clothing allowances. These morons are clueless and unfortunately the Nigerian masses are even more clueless. I weep for this bastaard of a nation, where medical professionals are forced to watch injured citizens wallow in pain till they pay up or die, I pity the nation where medical professionals are so severely underpaid that they must choose between survival and morality. Finally I pity the youths wishing for a better tomorrow,there's no such thing as tomorrow when today we keep making the same mistakes of yesterday, our politicians visit abroad for medical treatments,they know how the system works yet prefers the stone age system of Nigeria. My advice to all medical professionals is to not be discouraged but rather do your best and leave the rest.....shalom.
