You are hereby requested to visit NBTE website on www.nbte.gov.ng and click Downloads on the menu bar. You should then click …. on the list of downloads or type the following link on your browser: http://nbte.gov.ng/nbte_........ You are also to scan the under listed credentials as applicable to the position you applied for and send to the e-mail below. Please note that ALL the attachments should NOT be more than 3mb in PDF or JPG formats only.

i. National Diploma Certificate or equivalent
ii. Degree or Higher National Diploma (HND) Certificate
iii. Masters Degree
iv. Certificate of Registration with professional body
v. Indigene Certificate
vi. Birth Certificate or Declaration of Age
vii. Evidence of Proficiency in ICT

Your response should be submitted latest ……. of July, 2017.

I received an email from nbte requesting same, but it didn't come with my application number and i can't find my acknowledgement slip. i would greatly appreciate some assistance on how to go about it thanks.

Yes the template was in excel and you don't have to convert that. Only your supporting documents should be in Jpeg or pdf formats.
