*WHAT CAUSES A GOITER?* One of the most common causes of goiter formation worldwide is iodine deficiency. The primary activity of the thyroid gland is to concentrate iodine from the blood to make thyroid hormone. The gland cannot make enough thyroid hormone if it does not have enough iodine. Therefore, with iodine deficiency the individual will become _hypothyroid_.Consequently, the pituitary gland in the brain senses the thyroid hormone level is too low and sends a signal to the thyroid. This signal is called thyroid stimulating hormone (TSH). As the name implies, this hormone stimulates the thyroid to produce thyroid hormone and to grow in size. This abnormal growth in size produces what is termed a “goiter.” Thus, iodine deficiency is one cause of goiter development. Wherever iodine deficiency is common, goiter will be common. It remains a common cause of goiters in other parts of the world. GoiterHashimoto’s thyroiditis is a more common cause of goiter formation. *_This is an autoimmune condition in which there is destruction of the thyroid gland by one’s own immune system_* . As the gland becomes more damaged, it is less able to make adequate supplies of thyroid hormone. The pituitary gland senses a low thyroid hormone level and secretes more TSH to stimulate the thyroid. This stimulation causes the thyroid to grow, which may produce a goiter. Another common cause of goiter is Graves’ disease. In this case, one’s immune system produces a protein, called thyroid stimulating immunoglobulin(TSI). As with TSH, TSI stimulates the thyroid gland to enlarge producing a goiter. However, TSI also stimulates the thyroid to make too much thyroid hormone (causes hyperthyroidism). Since the pituitary senses too much thyroid hormone, it stops secreting TSH. In spite of this the thyroid gland continues to grow and make thyroid hormone. Therefore, Graves’ disease produces a goiter and hyperthyroidism. Multinodular goiters are another common cause of goiters. Individuals with this disorder have one or more nodules within the gland which cause thyroid enlargement. This is often detected as a nodular feeling gland on physical exam. Patients can present with a single large nodule or with multiple smaller nodules in the gland when first detected. Thus, in early stages of a multinodular goiter with many small nodules, the overall size of the thyroid may not be enlarged yet. Unlike the other goiters discussed, the cause of this type of goiter is not well understood. In addition to the common causes of goiter, there are many other less common causes. Some of these are due to genetic defects, others are related to injury or infections in the thyroid, and some are due to tumors (both cancerous and benign tumors). CEREBRAL PALSY (CP) Cerebral Palsy is a heterogeneous group of conditions that results in permanent, non-progressive motor disability. It may be due to perinatal hypoxic insults, developmental brain abnormalities, genetic conditions, traumatic or infectious causes. The insults may occur at any time during gestation resulting in variations in the clinical patterns observed. The consequence is a major burden on the patient and family socially and financially. It is estimated that about 2 per 1,000 babies are born each year with CP. Improvements in the neonatal care had little or no effect to date on this incidence. There are several classifications used for CP depending on different elements of the motor function. The Gross Motor Function Classification System (GMFCS) is adjusted to age and reflects the degree of independent activities and functions. Another way to describe and classify CP is by the number of affected limbs (e.g. hemiplegia, hemi-paresis, quadreparesis, etc.). A third method is based on the presence or absence of spasticity, coordination, and the muscle tone. Spastic hypertonic, or non-spastic, pyramidal or non-pyramidal based on the patient coordination. This description usually reflects the site and degree of pathology. The non-spastic, pyramidal type can be further classified as ataxic or dyskinetic types. In addition to these types and descriptions there are cases with mixed manifestations involving several features described above in associations with other elements like Autism. This debilitating disease seemed a logical target for stem cell therapy to repair the neurologic defects, stimulate the endogenous stem cells to repair and regenerate and reestablish the lost balance between the different neurotrophic factors. Asthma treatment with Stem Cell. Bronchial asthma treatment. The main manifestation of sinusitis is inflammation of one or more of the paranasal sinuses and includes the following symptoms: genyantritis, sinusitis, and sphenoiditis, ethmoiditis, depending on the type of sinus that is involved in the inflammatory process. This failure of the sinuses develops the pansinusitis that occurs when all the paranasal sinuses are inflamed and irritated. The main symptom of sinusitis is nasal congestion, which often becomes a huge and unsolvable problem. The most common causes of acute sinusitis are acute respiratory infections (ARI), influenza virus, rhinovirus, adenovirus, staphylococcus and other pathogens. But what do you know about therapy of mesenchymal stem cells? Complications of acute sinusitis may be a chronic process leading to frequent exacerbations, which may be the trigger in the development of allergies. Over the past few decades there has been marked a sharp jump in the allergies incidence. Today there are 5 subdivided variants of hypersensitivity reactions, one of which is responsible for allergic hyperreactive immune response. Allergies occur when the foreign protein – allergen – enter the body eliciting a protective immune response. The result of an allergic reaction can be allergic shock, vasomotor rhinitis, urticaria, false croup, angioedema. The most common inflammatory disorder is bronchial asthma caused by bronchial hyperreactivity. The basic mechanism of the disease is the narrowing of the bronchi during an asthma attack, accompanied by bouts of breathlessness, and in severe cases, development of asthmaticus status. Development of allergies: Allergen enters the body. Antigen-presenting cell takes up the allergen molecule and presents its epitopes, through the MHC II receptor. The activated antigen presenting cell then migrates to the nearest lymph node. T cells (type of lymphocytes) are activated and recognize the allergen. They give the instruction for the T cell to differentiate to Th2 cell. B cells (type of white blood cells) recognize the allergen through the activated Th2 cell. B cell would be activated. Differentiate into plasma cells, at which point they would actively synthesize antibodies of the IgE isotype. IgE antibody recognizes epitopes of the allergen molecule, circulates around the body through the lymphatic and cardiovascular systems and finally binds to its FcεRI receptor on mast and basophil cells. When the allergen re-enters the body it binds to the IgE, which is on the cell surface, resulting in an aggregation of the receptor causing the cells to release pre-formed mediators. One of these mediators is histamine which causes the 5 symptoms of allergic inflammation: heat, pain, swelling, redness and itchiness. Another mediator is IL-4, which affects more B cells to differentiate into plasma cells and produce more IgE and thus the vicious cycle continues. It is therefore necessary, as early as possible, to carry out sanitation of foci of chronic infection - treatment of acute and chronic sinusitis, which is difficult to treat with traditional methods. Unfortunately, the treatment of allergies and asthma by classical methods nowadays is also ineffective without stem cell therapy.
