The IMM 5409 (common law statutory declaration form) after filling, how do I do about it, should I take for notarize? Am marry legally do I need to fill this form, am a student who want to apply for study permit and thsane time for my wife open work spouse

You don't need any other form. The signed consent form you sent to the person assisting you is only form that is required for this process.

That form where you tick whether you need to take a medical exam or not. Its part of the CIC documents to be uploaded if applying online

Hello house, My SOP is a 12 page document which I know is many. But the reason why this pages r many is cos apart from the normal procedure of writing an SOP, I farabale wrote down points to tackle my previous refusals. Please note I have been refused twice with four reasons each and this is my 3rd application, and I don't wanna be refused the thrid time that's the reason why my SOP is this many. Now to my question, 1. should I leave the SOP like that cos I don't think I wanna remove or delete any sentence from my SOP. Or I should just split it into 2 parts. I.e SOP and LOE.(I know they r both similar). My plan is to write ma SOP and then write ma points to tackle my previous refusals in my LOE. Ur input will be highly appreciated..

B.Sc Industrial Engineering, University of Ibadan. (First class). Graduated January 2017

MSc admission to University of Waterloo, Systems Design Engineering. $36,635 per year funding ( comprising both RA and TA) and Dad also transfered 3M naira to me. Tuition $24,000

Had work experience with 4 different engineering firms(internships).

Submitted all CIC forms
Police Certificate
Employment docs
Dad's Employment and retirement docs
Landed property
Dad sponsorship letter
my CV
Perfectly crafted SOP
My SOA and Dad's SOA
My UG transcript
whole family birth certificates
NYSC documents.

Almost ready for 2nd application online

1294 form (total cost of 19,000 CAD)i indicated I was previously refused


My maternal uncle is my sponsor he has been to the US twice
Sponsorship letter (he indicated he has six children)
Affidavit of sponsorship

Business closing account of 13million after he transfered 4million to me
Inflow of 72million and outflow of 59million

His personal savings account of 4million
I already paid 1 year tuition of 4833cad before my ist refusal( I said my sponsor paid)
I have his passport data page my mums birth certificate
My mums marriage certificate.

Sponsors business documents (letter of incorporation, memorandum and article of association, tax clearance for past three years)

My landed document 100*100 deed of transfer
My mums two landed documents deeds of transfer
Note: no receipt of purchase cos we weren't given, is this OK considering there is no estimation of current land value?

Family business letter of incorporation(my mums current business)
Pictures of family house under construction with building plan.
Siblings birth certificate

Did you complete another postgraduate programme in 2017 or you were working before getting your first degree.

VO explicitly mentioned that you were working since 2011 and graduated in 2017.

Otherwise, were you working with a ND before getting a bachelor's in 2017?

All those statements mentioned casually may need to be addressed and clarified.

Yes i was working since 2011 with my HND had my Bsc. In 2017
I wrote it out in my previous sop
I dont think the vo even read my previous sop then
How can i address this now?
Do you think i should apply alone these time?

Applied March 10
Medical request April 30th
Passed medical Early May


No final decision yet and this is my 3rd months of application..,
