In Orphans and Vulnerable Children (OVC) program, the national HIV risk assessment checklist is often administered to vulnerable caregivers and children to be able to evaluate their vulnerability status and their risk of contracting HIV.
One thing service providers should note is that the HIV risk assessment checklist is not be administered to people that are already HIV positive. People living with HIV are not meant to be risked assessed because they have already contacted the virus. At that point, they don’t need to be risk assessed whether caregiver or OVC.
The national HIV risk assessment checklist is meant for those who are negative but maybe at risk of contracting HIV.

Conditions for administering the National HIV Risk Assessment Checklist
Some of the reasons for administering the national HIV Risk Assessment Checklist are listed thus:
If the child’s parent or caregiver/household head is chronically ill.
If the child has an HIV positive confirmed caregiver or household head in the household.
If the child or caregiver has not been tested in the past 6months.
If all members of the household’s HIV status is unknown.
If the child has a recent history rape or any other sexual abuse with the past 3month.
If the child or caregiver has been seriously ill in the past six months.
If the child lives in a child-headed household.
If the child is living at least one member of the household who is HIV positive.
If the child is active.
If the child is drop out.
Guidelines for filling the national HIV Risk Assessment Checklist.
The first thing that must be done before administering the HIV risk assessment checklist is to first give the caregiver or household of a particular orphan and vulnerable child the consent form to sign first. The consent form is given to parents o sign, whose children are less than 15years of age. The consent form is for the caregiver to grant permission and give approval for his/her ward to be tested.
