Check your visa receipt, there's an email address there. Send it to that email, include your visa applicant number

I applied for the health insurance certificate and I thought I already got it but it looks like what I was sent is just another form even though I already filled and submitted one. I just need to know what the certificate looks like and if I have the right one. I have just few days till the end of enrollment and this is the only thing delaying me. Already contacted AOk for confirmation but I haven’t gotten a reply yet.

Please if you have gotten your certificate from them recently please send me a message so I can show you what the one I have is like and confirm if that’s what’s it should be or not. Please guys, this is urgent. Thank you!
