Actually, there have been record of air collision accident
The rate of collision is very rare due to the fact that pilots are being monitored by ATC, air traffic controller and computer installed in each plane called TCAS, traffic collision avoidance system.
This two must fail for mid air flights collision to occur

There's a device that alerts them of possible collision. Check for "Japan Airline near collision" on youtube

Insurance companies are in real mess ... a dent on an airplane may cost thousands of dollars. Let alone an accident ? Hmmm �

They have collided on a few occasions before. Watch Air Crash Investigation on National Geographic to learn more.

Technically, the air traffic controllers at various airports detect & guide the numerous planes flying in & around their airspace such that they don't collide with each other, that's one of their primary jobs.
In addition, aircraft now have an inbuilt system called TCAS, Traffic Collision Avoidance System, which warns them if any other aircraft is coming close to them while in flight, and guides them to avoid the oncoming aircraft.
