This "the lady is the solely determinant of whether the marriage should occur or not" holds true where a woman is involved with a nonaristocratic man. Aristocratic men have always determined whether a marriage will occur or not because women of all strata always do everything possible to be in the life of aristocratic men, not the other way round.

It's because a man is a family unto himself and a woman is not. While titles like "Man" "Mr" etc are a matter of age for a male, the female can only get the title of "wife" and "Mrs" through marriage. A woman is therefore wedded to a man on a wedding day. A wedding day is not a man's day, it is the woman's wedding day, hence her name comes first. it's same in Igbankwu.
