You don't need to go through Cotonou for now. You can easily go through Kenya airways from Lagos,Nigeria.
But I will advice anybody coming to search for job to shelve the plans at the moment and exercise patience for the work visa restrictions against Nigerians to be resolved by both government to avoid issues.

Wait, I want to understand you, do you mean that someone can fly from Nigeria to Dubai through Kenya airways now?

For someone going to Dubai for medical purposes (surgery), how does the person apply? like what type of visa do you apply for? also Is it possible to now travel to Dubai directly from Nigeria or do we still need to go to Cotonou?

For someone going to Dubai on medical grounds you do not need a tourist visa in order to get a medical treatment. It is your sponsor, that is the hospital where you will receive medical treatment that is in charge of arranging your visa/patient entry permit as it is called.
The hospital has to be licenced and registered in the UAE for them to be able to sponsor your visa/patient entry permit.

It is possible to travel with Kenya airways from Nigeria to Dubai irrespective of your status, all you need is to call Kenya airways and state your case, you will be told all the necessary requirements you need to meet for you to travel from Lagos, Nigeria.

And note that currently there is an embargo on tourist visa for males below the age of 35 by the UAE government, though there are ways your sponsor can bypass the process.

You don't need to go through Cotonou. Just book your flight through Kenya airlines. Before making payment call Kenya airline customer service to know the basic requirements to fly from Nigeria.
