Just to put things in perspective for people. The normal voltage in your house that can kill is around 220V, now these guys were exposed to 11,000V.

That is instant death na... I do not want to imagine how their last moments were like.

It is a legal requirement to have Risk Assessment before such a work can commence, that document gives room to identify all the likely hazards and also provide means of control so far as it is reasonably practicable.
Method statement is also a recommended guided on the sequence of the job...
It is completely unacceptable for workers to die while at work...
In the practice of managing Health and Safety, there are three reasons why accidents must be prevented, Moral, Financial and legal reasons.
Our people come to work to provide for their family not to be harmed by the work they do...

Consequences of this breach of law is prosecution of organization, license cancellation, fines...
