So far so good. But since the patient is elderly, the main work should lie with a dedicated caregiver, who lives with him/her.

This caregiver will be able to ensure dietary and other lifestyle/non-pharmacological control, as well strict compliance with prescribed medications

He/she should also be responsible for regular check and documentation of the blood sugar, monitor for complications and ensure hospital visits/follow up when needed/necessary

There's this drug I know my boss uses - Treviamet. He uses it to manage his diabetes. I will advise you check with a physician if it is safe to use for your condition. Goodluck.
