Good morning, the challenge you are facing may be as a result of many factors including
1) bad drainage on the tank slab hence water retention which then seeps down through your porous slab.

2) Quality of workmanship,supervision during slab concrete work.

3) Rain water and water from your tank can also seep down through the tiling as the cement grouting in-between the tiles wear off after a while.

Solution
You have to felt the whole slab including some upstand areas.
