CareerParity, Equity Or Slavery: The Dilemma Of The Nigerian Health Sector Wage System by cutechioma1(op):
Parity, Equity Or Slavery: The Dilemma of the Nigerian Health Sector Wage System.

By Aniey Obot.

The Nigerian health sector has been tumultuous for some times now. It is either members of the Nigerian medical association (NMA) are downing tools using its affiliate, the national association of resident doctors (NARD) as her war horse or the members of the joint health sector unions (JOHESU) are on strike paralyzing health services in the country. Presently, health services in the country have been paralyzed for over three weeks now due to the ongoing strike by members of JOHESU, comprising all health workers aside medical practitioners.
One of the reasons JOHESU, which is a marriage of necessity and convenience of all the registered trade unions in the Nigerian health sector, is on strike, is the wage system in the sector. They are pushing for an equitable and fair wage system, describing the present wage system as a sort of slavery. The NMA which is the umbrella body for all medical and dental practitioners in the country, is seriously pressurizing the government not to grant JOHESU’s demand because according to her, JOHESU is asking for pay parity with doctors. According to NMA, this is against international best practices. This was echoed by the ministers for health and labour who incidentally are members of the NMA.
Parity, Equity and Slavery! These words have been on the front burner of discussions within the sector. NMA is rejecting parity, JOHESU is denouncing slavery and asking for Equity. To understand the discussion, these words must be defined. We take the definitions from dictionary.com. Parity is defined as the state or condition of being equal, especially as regards status or pay. Equity is defined as the quality of being fair and impartial. Slavery is defined as a condition of having to work very hard without proper remuneration or appreciation.
The story of the wage system in the Nigerian health sector is a very long and complex one. To avoid boredom, I will concentrate on the evolution of the present salary structures in the sector namely CONMESS and CONHESS. In 2009, the federal government approved CONMESS for medical and dental practitioners alias medical doctors and CONHESS for other professionals and workers in the sector. These salary structures are gauged against the popular grade levels in the civil service. The basic tenets for these salary structures are that, every worker on the same grade level should earn the same basic salary and common allowances now called consolidated salary. Peculiar allowances should be paid to different professional groups as approved. Different professions have different entry level according to their years of training and the work they do. Any future adjustment must be done on both tables. These they say is to maintain an equitable pay relativity (this is a topic for another day). The NMA and JOHESU where actually unhappy with the new salary structures, because of the pay gap. NMA claimed it was too narrow, and JOHESU claimed it was too wide, they accepted the salary structures and forged ahead. Surprisingly, in 2014, against the tenets of the 2009 agreement, the federal government approved an upward adjustment of CONMESS to the satisfaction of NMA without an equivalent upward adjustment CONHESS.
The real bone of contention here is the issue of pay gap between the medical doctors and other health professionals. To appreciate the cries of JOHESU, an analysis of the wage system in the Nigerian health sector is necessary. For the analysis, a nurse will be used to represent an average health professional. If a post NYSC medical doctor without any postgraduate certification or experience and a double qualified nurse (a registered nurse who has a post graduate training as a midwife) are employed the same day into the federal civil service, the doctor will get employed into the service on CONMESS 3 which is Grade level 13, while the nurse gets employed in the service on CONHESS 7 which is grade level 8. At the end of the month the salary of the nurse will be less than 30% that of the Dr. The nurse will need to work for about 15 yrs to get grade level 13 which is the entry level for a post NYSC medical Dr. To earn the same as a newly employed Dr, the nurse will have to work for about 21yrs assuming he/she passes all the promotion interviews!
One of the reasons JOHESU is on strike is to narrow this pay gap and maintain the 2009 relativity, all they are asking is for people on the grade level to earn the same consolidated pay( basic + general allowances) whetherDr or nurse. Earning the same consolidated pay is not the same as earning the same salary. Remember it takes a nurse about 15yrs to be on the same level with a newly employed Dr.
All JOHESU is asking is let an average health professional after spending 15yrs in the service to earn the same consolidated pay with a newly employed doctor.
It is so surprising that the NMA both ministers of health and labour are calling this demand an attempt to achieve pay parity with doctors. It is evident they either don't understand the meaning of pay parity or the relationship between the 2 salty structures. I will not want to believe they are mischievous as only mischievous people will go on national television and dish out what they know are lies to win sympathy from the public.
Now the onus is on the public to judge if JOHESU description of their ordeal in the hands of our health managers as slavery and their demands for an equitable, fair and just wage system is tantamount to requesting for pay parity with doctors.

My friend who was earning peanut as a Doctor in Nigeria, first left to the middle East, he got a job and the starting salary was 1.2million per month which to me was a peanut considering what other doctors are earning, he later wrote some exams, I think it is called Plab or so, as I post this, he got a fantastic offer in England, he is working with a hospital in England and doing very well, when you point out the pitfalls of working in a shithole jungle, they will call you names, if you are a medical doctor and you are still working in Nigeria, Na you sabi sad
And to think that when he gets there, will definitely and respectfully work with all those he calls inferior down here. He will meet and older nurse, lab scientists, pharmacy and greet first because that is what everyone does. The truth of the matter is that no one has respect for each other's profession and this is what is causing collapse in the health sector

SmallmebigGod:
I was discussing with some of my colleagues today.
What are we doing here?
Looking back at the thread of things in Nigeria.
Till Sometimes around late 80s and early 90s the on thread was to start your own hospital after NYSC or work as Medical officers.
Gradually the drive for residency training program started growing and having more medical schools gives the consultant cadre much job so residency training became lucrative
Onset of Conmess and all the adjustment makes Residency training almost the only career path that Nigeria doctors know.
Sadly in 2014 when the then minister of health and some 'medical elders' asked the Presidency to sack the resident doctors, this move reactivated our brains and we started searching for an environment that is more rewarding and less depressive like Nigeria health sector. Many people that has been seeking to move before then also hasten up their processes. I remember someone on Nairaland that time sent a documents to us on how to qualify for practice in about 48countries, UK,us,Canada and Australia inclusive.
And then comes intense hatred and fight by Johessu worsening the poor working conditions.

Medical students no longer plan to practice in Nigeria, they have started writing qualifying exams to practice in US UK etc immediately after their Induction. Any other place apart from Nigeria.
What are we still doing Here?
Brain drain we are seeing now is just a tip of the iceberg compare to what is to come.
Just last week 2 professor of medicine and two other senior consultant independently confided in my that they are planning to relocate
My dear everyone in Nigeria wants to relocate cos of greener pastures, its not exclusive to doctors alone. Its all about the money and comfortability. If that is all the reason you read medicine, pls don't frustrate yourself by staying back. But I bet you some people out there are still reading medicine because they care and want to better the lives of humanity. I mean why do we still have doctors without borders, I'm sure it not because of the money.

I'm not a Dr but i know the pay system is in ratio even in canada, usa n Australia so what exactly does johesu mean by salary increase for the bodies.? Is it to remove the ratio or top up 4 all in same ratio?
A medical doctor entering civil service suppose to be one level ahead of other health proffessionals because their years of training is one year ahead of other health proffessionals ,As we start on conhess 9 ,they suppose to start on equivalent of conhess 10 ,but now, they start on equivalent of conhess 11 ,this is greedness ,Relativity is the difference in call duty .A medical doctor receives 200% of his basic salary as call duty whereas other health proffessionals receive 40% of their basic salary as call duty ,That is why they don't want conhess to be adjusted

the doctor is the leader of the healthcare team. Fact. who will take responsibility if something goes wrong with a patient, definitely not,JOHESU. who gets litigated, definitely not JOHESU.

You people want the benefits but are not prepared to take the risks that go with it
And you think a Med lab Scientist have not been fired before because of an inconsistent lab result due to Dr's interference, a nurse arrested before due to the negligence of the Dr. When a case of litigation comes up, everyone involved in the care of the patient is called upon from the Dr to the nurse down to the ward orderly that mistakenly forgot to greet the patient. The risk is high for all health workers.

please always arm yourself with information before talking.
I have been an onlooker silently viewing this threads for days but couldnt stomach It any longer.
This advanced world have CMDs who is a doctor, always a doctor, then a board consisting of other people, which is headed by the CEO or board chairman [as is obtainable in nogeria[]..so its the same thing here too, the board or CEO os just an oversight function.
But the management is left to the CMD.

And finally of the other health sector workers ever though of and ever wanted harmony, whats the purpose of a "joint health.. .[JOHESU]" without doctors, is that not ironic.. "joint".
My dear for your information, JOHESU was formed to have a stronger force against our employers (govt) and not to fight Drs. If doctors choose to join, the door is open. Not too long a ago, all health workers including the Drscame together to form medical and health workers union of Nigeria( MAHWUN) and guess what they left because they felt its inappropriate to fight beside their inferiors.

OgaInnocent:
Point of correction. Doctors can disrespect other health professionals because they are not their rivals. But if there is disrespect by nigerian doctors to JOHESU, it is simply reactionary due to their (JOHESU) continued challenge of the doctors. Imagine a graduate of medical laboratory science hanging stethoscope on his neck and saying he is a diagnostic doctor. Can u imagine that? Does that happen in your civilized setting?

In Nigeria, every one working in the hospital is answering a doctor. Doctors can't just fold hand and watch them. So any action by the doctors is reactionary. Otherwise, what does a doctor gain by victimizing a nurse who has done him no wrong. But when a nurse starts claiming he /she can do what d doctors can do...I wonder!! My brother, just reason that.
I agree with you, we should stick to our speciality but what of doctors that wants to prove they can do other people's job. Eg, a Dr will come into the lab and start working on the bench all in the name of a pathologist. Do you know many times they commit blunder just to ascertain their authority. You can't spend 6yrs + 1yr horsemanship to prove that you are better than others just to end up fighting to see that bacteria in the urine.
JOHESU is not looking for equal pay with the Dr but equal rights, to be considered as a key part of the patient's recovery plan. If the Drs believe they are the head of this great team, please in the name of true leadership should carry everyone along. As far as am concerned, the most critical member of this team is the patient because without him/her every other person cease to exist.

HealthRe: Speech Therapist In Owerri Needed For My Son by cutechioma1: 5:35am On Mar 07, 2018
That speech problem is the characteristics of 60% of 3 going to 4yrs. It is as a result of starting early to watch cartoon which kind of distort their ability to speak early because of the fastness of the cartoon characters talk, when they find it had to catch up they tend to babble. My son is like that and even most of my neighbor's son his age. My advice is stop all carton for now and only show educational videos that speaks slower. There is no cause for alarm.
