The form can still be used. If u check the form it states that the signed signature of non appearing parent is valid for 90days.

@ favoredb, you don't need an agent at all. I got my baby's document in five days. Just go to a close by post office fix an appointment and fill the forms which u will return on the appointment date along side with baby's bc, parents passport page, and 1 psspt photograph of the baby. The post office collect it for further processing and ur baby's passport and ssn will be sent to u in less than 10 days. Pls be informed that the original bc will be collected but returned back to u along side with the baby's passport book and the ssn will be sent a day after u receive the passport book. They usually don't send passport book and ssn together in one package becos of security reasons. For Nigeria pssprt u can get it done same day.

uchetobi: Call them and tell them you want to add am infant to your return leg. They will effect the changes then tell you how much it costs

How do I go about that. Does it mean I have to bring the baby back to USA on the return date or that portion is meant for minors above 16years. Pls I need an urgent response.

uchetobi: How far along will you be then? Arik does not carry pregnant women more than 30 weeks old and they won't accept your fit to fly. They will send you upstairs to be examined so make sure you will be less than 30weeks then

Arik does not carry Peggy more than 28weeks pls call their office to reconfirm their travel policy before buying your ticket.

uchetobi: I flew British air. I dint have that return ticket problem. I think it's common to American airlines although Tha babe also had the problem with airfrance.


The ticket is probably that high because it's also summer. I think the price will remain the same as the day you will buy at the airport!
