You can only get differentials from the symptoms, you need an actual test to be sure of what exactly to treat You are not far from the truth. However not everyone has the luxury of running test. Plus majority of people with malaria like symptoms are actually suffering from malaria, especially in a mosquitoe prone areas like Nigeria. If you look at their plot very well it was very local, if you are a medical practitioner you'll understand my point. |

Akorkor: True!! A lot of times I chat up with my doctor when I was even pregnant to prescribe something for me after telling him all the symptoms because it may be in the middle of the night. Then next morning I go physically to hospital for checkup.
Doctor call is in - thing now.
Malaria drug is OTC and is nothing new. I didn’t watch the commercial so I can’t give my ratings
However, amartem advert shows only one guy that was shivering and was even scared of injection before he was prescribed malaria drug.
With this, I don’t know which team fit that That guy was already in the hospital. They wanted to go Intramuscular, but the guy couldn't tolerate due to obvious phobia for needles. So the doctor had to prescribe the best oral medication he can think of. |

Honestly, i am not an advovate of over the phone physician care. But the world is evolving and its the next phase of medicine. I too dont advocate for over the phone consultation. But in Nigeria its very common especially from friends and family members. |

Maybe he doesn't know there is poverty in the land and not everyone can afford ordinary malaria test. I have been to different angle of the world and sometimes MD don't conduct some test if the symptoms you shows its so obvious unless as a patient you actually want to confirm before prescription. Doctor visit alone in US is $100 not testing oh. Now if you want to carry out some test you know how much that is.
Nigeria is cheap but people can't still afford it

You say you’ve attended hospitals where tests where done before you were treated. That is not abnormal and that is even recommended especially in private centres where as a doctor you make money for your institution. Working in a teaching hospital, I have learnt to understand my patient and learn empathy towards their pocket. The NCDC and Min of Health encourages prompt RDT before treatment of malaria but allows treatment commenced when you identify the classical symptoms: Fever, headache, chills and rigor. As a patient, you are also allowed to self commence as much as you contact your doctor on the 3rd of use if no changes.
I guess you want to educate me on differentials of malaria...lmao. I spent 8 months reading Robbins and Cortrans reading on all pathologic conditions known to man. I am sure I would name 40 other conditions before you get to 5. So rest.
Educate yourself on the paragraph about losing one’s license. It’s ludicrous. As a medical student in Africa, you are drilled on making clinical diagnosis without the help of investigations. Its our strength and why we are admired all over the world. If you think MDCN would take my license for doing something i am paid to do, you are the one needing a CT Scan then.
Your last paragraph shows your mentality. Nigerian doctors are sought after across the world. Bring cases of anyone denying his/her patients confidentiality without being punished. The only place I get malaria prescriptions without testing are chemist shops or independent pharmacies .
And there are host of diseases that have malaria-like symptoms . HIV for one can often come with malaria-like symptoms two weeks after infection .
Hepatitis a also has malaria-like symptoms at the initial stage.
I don't know about you , but a doctor who just recommends a prescription simply because the disease is supposedly an endemic is highly irresponsible and should lose his medical license.
But then again , you say this only happens in Nigeria . A country where patient confidentiality isn't even respected . I guess anything goes . [/s] Quiet. So you go to hospital or a lab to run test everytime you have malaria? Do you know the meaning of OTC medications? I repeat quiet!

mycar: Not advisable in a professional settings to prescribe without lab test, if you must do that, you must tell them that if symptoms persist after two days, the patient should see a doctor but team two seem to haves changed their script. It is advisable, looking at the scene they painted, a poor dirty neighbourhood. What are the disadvantages compared to the advantages? Remember this is Nigeria not europe where malaria is not endemic, and their setting was a local setting where people dont have money for medication not to talk of test. Then besides the test they had scenes in their drama that proves it's likely malaria. They'll definitely see their doctor if after completing medication no improvement. |

IgANephropathy: Malaria is endemic in Nigeria. Peculiar symptoms are likely to be malaria than any ailment. As a doctor, especially in rural settings, you prescribe antimalarials if your patient presents with classic symptoms even without tests (especially high risk patients: Pregnant women and Under 5 children). Although the mass introduction of RDTs by the Fed Govt has made test easier, even though that itself isn’t reliable.
My point is: you can prescribe antimalarials without investigation in Nigeria. I'm happy to see highly cerebral fellows on this forum. Thank you for explaining to the thousands of dumb fellows on this thread.
