I'm required to use my Birth Certificate in conjuction with some of my other documents for educational purposes, and I just found out that the Surname used on my Birth Certificate is different from the one on all the other documents.

So, someone advised me to do a change of name affidavit.

What are the processes entailed in getting an affidavit? And where can it be done here in Ikorodu?

Sir, I think you should have the person that received the phone to power it on and operate it. So that you can plan a return before seven days if it is faulty.
