It is not possible to advise you further based on your vague description because "chest pain" can be caused by a multitude of things. The most worrisome of which is the heart but a majority of the time is due to something else. Such as GERD (gastroesophageal reflux), musculoskeletal strain (especially if the area is tender when you push on it), chronic cough to name a few of the most likely culprits

A couple of things. The fact that it has been present for that long is quite unusual (since he was 8 months till now that he is 3yrs old). It never improved or resolved in between? This type of rashes in children are typically viral. Particularly given the distribution things like herpes zoster, unilateral laterothoracic exanthema come to mind. However, this typically resolve on their own and treatment is aimed at shortening the duration and symptom management.

I would recommend applying calamine lotion or 1% topical hydrocortisone to help with the itching and decrease ongoing irritation. You try to take him to a dermatologist if you can find a reputable one for further evaluation. Someone needs to take a sample from one of those vesicles for further testing which might help make the diagnosis.

Hello doc. My 3years old boy has this blisters on his stomach. Has taken him to two govt hospitals. But all their prescriptions has not solved it. what could be the cause and remedy.
Sorry to hear about the difficulties your little boy has been having. I have a few follow up questions to better understand whats going on in order to try to provide the most accurate response possible.
-When did you first notice this? how long has it been there?
-Did it always have that red appearance or did it change after some time?
-Is he having any other associated symptoms such as fever, nausea/vomiting, itching around that site or pain etc?
-Is he up to date with his immunizations?
-Can you please take another picture that shows most of his trunk from neck to lower abdomen (please don't show his head) so I can get a better sense of the location and distribution
-What type of medications has he been given so far?

I can certainly understand your perspective on this. The best way to pursue a research assistant position is to reach out to the specific departments at your universities of interest directly. Introduce yourself concisely, detailing your background and purpose of your inquiry. The more academically accomplished you are by virtue of current research publications and stage in academic training the higher your chances would be in securing a position barring a significant immigration related issues. These departments typically have funding appropriately for each years spending and the sooner you reach out to them the better as local applicants have the benefit of home field advantage. It is also important to utilize any contacts you might have at any one of these institutions as they are more likely to respond favorable to recommendation from someone they know personally.

There are also other national programs from the National Institutes of Health, Howard Hughes medical institute (2 major ones that come to mind) that are competitive but typically are excellent programs with great exit opportunities. The key here is to be optimistic, and persistent because you will likely have to knock on many doors before one actually opens.

Paranoid schizophrenia is a type of schizophrenia based on the type of signs/symptoms being predominantly displayed. In this case paranoia is a hallmark of this in addition to meeting the classification for schizophrenia which typically involves ongoing psychosis.

Regarding your main question which is treatment. Schizophrenia is typically managed with Antipsychotics which are classified by their generation (i.e 1st generation, 2nd generation based on when they were introduced to the market with 1st generation typically having a worse adverse effect/side effect profile). However, that is where my recommendation will stop because this should actually be closely managed by a psychiatrist as this are powerful psychoactive medications that should be closely monitored. In my prior trips to lagos, I have commonly seen this medications on the shelves of (though not sure of their authenticity) the local street pharmacy along with cancer medications. When I asked the seller what they were used for his response was comical and actually scary to consider people are buying this off the street.

I certainly understand your concern and it is great that you are being proactive and paying close attention to your health. I agree that it is concerning that you are consistently getting systolic blood pressure numbers (upper number) above 140. Two independent readings above 140 as you have described meets the definition for hypertension. However, the other thing to consider is "white coat" hypertension which is not really hypertension. This essentially means your blood pressure is only elevated in the medical setting like a doctors office (hence the term white coat). In this case, I advise patients to obtain a home blood pressure cuff and check their blood pressure at home and to see if it is still consistently elevated. Often times, it normalizes on this environment.

With that said, if the diagnosis of hypertension was confirmed then efforts would be made to make lifestyle changes particularly if it's only mildly elevated. This means weight loss, exercise and maintaining a healthy diet. Often times, this is enough to manage this level of hypertension. If still unimproved then we can pursue secondary causes of hypertension given your young age and explore therapy with medications.

The key here is that blood pressure medications is not the first step!

It is necessary but not mandatory including the one taken after delivery. This is because if after birth and your baby's blood group is also type Negative, then you don't need rhogam.

Some people like giving the injection at 28-30 weeks and after delivery others just give at delivery while another set of people will insist on knowing the baby's blood group before giving. Infact because of the cost many prefer the last two options.

However, if you experience bleeding or fall while pregnant doctors will advise you get the injection unless you have determined that the baby you are carrying in your womb also has a negative blood type.

LVH t abnormality in high lateral leads
These findings require a bit of context. Why was this EKG obtained? where you having symptoms such as chest pain with activity or shortness of breath with activity? heart palpitations? the answer to these questions are more relevant to the "t wave abnormality in high lateral leads" that were reported. If this is happening in the setting of active chest pain then it is concerning. If not then its probably due to the associated LVH that I will describe below.

LVH is an abbreviation for Left Ventricular Hypertrophy. This essentially means the heart muscle is getting larger due to elevated systemic pressures (Hypertension) or less often an issue with a heart valve causing increased pressure in this chamber. This finding is common in people with long standing hypertension and certainly puts you at risk for heart disease due to increased demand by the heart. The heart just like any other muscle in the body gets bigger when it is faced with increased work. This is essentially the premise behind weight lifting and various other forms of resistance exercise. However, the heart is the one organ that you don't want to get bigger as this compromises its normal function.

I am guessing you are probably a bit older and had hypertension for a while. It is important that you make the necessary lifestyle changes which includes maintaining a healthy diet rich in fruits and vegetables and low in salts and fat. As well as exercising 4-5 times weekly for ~45 mins. (higher frequency is better than 1-2 very long work out sessions). You should also follow closely with a primary care doctor to ensure you are on the appropriate antihypertensive regimen. If you are having symptoms suggestive of heart disease such as chest pain, shortness of breath or palpitations with activity (walking up stairs) then you might need to see a cardiologist.

How long has this been going on and has it always been associated with your period? If so, then this is likely due to fibroids especially with the reported associated back pains as well. Fibroids respond to the hormonal stimulation that is associated with the usual menstrual cycle which leads to inflammation and resultant abdominal pain. Often times, the associated back pain is due to fibroids positioned on the posterior aspect of the uterus which essentially pulls back on it and causes some of the pain you feel.

This actually needs to be confirmed via ultrasound and a standard medical check up. This inflammation can be slightly suppressed with non steroidal anti-inflammatory drugs but this should only be used for short periods of time due to associated adverse effects with long term use to your stomach and kidneys.

This will be a recurring theme in my responses but I believe the better question is why you are having "serious insomnia". What is the best treatment often only leads to treatment with medications without consideration of the actual cause.

Your insomnia could be due to a multitude of issues and it takes a good history to figure out why you are having insomnia.
-Do you have issues falling asleep? or staying asleep?
-How long has this been going on?
-What age range are you in?
-Difficulties with mood? feeling more down than usual? stress at work? stress at home?
-Sleep hygiene: do you have a sleep routine? TV in the room? electronic devices before bed? what type of drinks before you go to bed (soda, coffee, alcohol)?
-Nightmares? palpitations/heart racing? racing thoughts?

You catching my drift?

I can easily put you to sleep with medications...it just takes the right combination. The extreme of that would be the Michael Jackson story where essentially I can sedate you. However, this does not fix the problem but rather only exposes you to a host of adverse reactions and consequences. Think Michael Jackson!

I would just ensure you are maintaining a healthy diet rich in fruits, vegetables, a good source of protein with meats or fish an try to exercise to maintain good cardiovascular health. The basic 3 meals daily with healthy snacks and appropriate hydration. The concept of "weight" and what is considered "thin" is often quite cultural and less anchored in physiological understanding of the human body. One should aim to maintain a relatively healthy range BMI with particular emphasis on staying away from the range of obesity based on your height and weight in order to ensure great health outcomes!

So this is actually VERY VERY concerning and requires further evaluation as soon as possible. I preface this by saying I can only work on the limited information provided from having not seen this patient in person. However, in medicine what you just described is what we consider a RED FLAG symptom that typically indicates a SERIOUS underlying problem.

In this case you report an individual with known prostate enlargement that is now having debilitating back pain to the point where he can't stand. I assume a physician actually diagnosed this prostate enlargement by physical exam or imaging or someone arrived at this diagnosis due to urinary symptoms. When one thinks of prostate enlargement, the concern is typically either benign prostate enlargement or prostate cancer. Benign prostate enlargement (BPH) as the name implies is benign (not harmful) and typically requires close monitoring and management of the associated symptoms which are typically urinary in nature due to the associated anatomic changes.

Prostate cancer on the other hand is serious diagnosis but still has great therapeutic options when diagnosed early. However, as prostate cancer progresses or sometimes even before it is diagnosed it can spread (metastasize) to other parts of the body. A very common location of spread is to the BONE. Therefore, when someone has prostate enlargement and severe back pain as you describe, the number one concern is for prostate cancer and this requires prompt medical evaluation to rule out this possibility. This is even more important with spine involvement as this can lead to serious neurological complications such as paraplegia.

Sorry to hear about your ongoing troubles. So you state you have been experiencing recurrent malaria and typhoid symptoms which I will interpret as mainly fevers and gastrointestinal symptoms such as abdominal pain/discomfort, constipation, diarrhea and skin rash to name a few. The "troubles and noise in your stomach" seem to be more consistent with gastrointestinal symptoms which is more associated with typhoid.

I assume most Nigerians are familiar with the cause and treatment of malaria (mosquito borne parasitic infection with characteristic cyclic fevers)

Typhoid on the other hand is cause by Salmonella Typhi and is typicaly spread via fecal-oral route. Essentially passed in the stool and transmitted by contamination of orally ingested foods or liquids. Accurate diagnosis with a lab test and treatment should be sufficient. However, if you are essentially being reinfected by virtue of your recurrent symptoms and this is proven on repeat testing after an interval period of resolution then the problem is really with prevention. As I previously stated, typhoid is transmitted via a fecal-oral route , therefore hand hygiene and being cognizant of the hygiene practices and quality of the food establishments that you patronize is very important. So if you find that this is a personal ongoing issue, you might want to be more careful with your local "mama put" and restaurants that you visit because you might just be reinfecting yourself until you isolate the source. You can also read about "Typhoid Mary" who was a cook in the 1940s that notoriously spread typhoid to dozens of people.

I personally don't believe in just taking drugs/herbs/procedures etc instead of getting to the actual root of the problem which might be as easy as great hand hygiene (hand sanitizers, hand washing, minimizing eating out and if you do then ensuring healthy food preparation practices). Unfortunately, typhoid is prevalent in developing countries due to a combination of sub optimal sanitary practices in massive population centers. Prevention is key!

The general term for what you seem to be describing is a subcutaneous nodule. Obviously an actual physical exam would lead to a more accurate assessment. With that said, that is just an unbrella term for this. The underlying cause of this really depends on the the characteristics of this nodule and other associated signs and symptoms.

Is it mobile (does it move when you touch it or just stay in one place)? is it painful? when did you first notice it (always been there or just popped up recently)? any other associated symptoms? rash? or is it not bothersome and you are just concerned because its there.

The human body is quite amazing and some of these growths are actually benign/harmless and might even resolve on its own. It could also be an indication of an underlying issue.

It really depends on the type and details of the hormonal imbalance in question. There are certainly many options available for women having difficulty with conception with medical available today. Secondly, there are truly on a few "hormonal issues" that would completely limit conception. Please provide some more information regarding the specific hormonal imbalance you are referring to and perhaps I could more accurately attempt to answer your question

Some health practitioners are happy with the current trend (if they are not, they would have attempted to change business as usual mantra)

I hear you...your thoughts reflect the frustration and general sentiment shared by the masses due to many years of the status quo across sectors. I agree that that there is a significant amount of inertia within our system that threatens to prohibit change. However, I believe we can learn from our colleagues in the business sector and realize that change is always possible. Let's take the emergence of ecommerce in Nigeria for example. If I had told you 10years ago that you would be able to sit at home and order an item on your computer and have it delivered to you within 48hrs as billed you would probably say I was crazy..." for this naija where we dey? Lol". Considering we barely have a functional postal system on top of the logistical nightmare of the poor infrastructure and traffic. Yet a few bright minds from Harvard (jumia) and another visionary in sim shagaya (konga) through hard work and creativity have made this a present day reality.
Once the consumer is exposed to quality product or experience they will continue to demand excellence which necessitates competition in the market to continue to deliver at the new expected standard.

2 years from now, if you walked into my practice and received excellent customer service from the minute you came through the door in an aesthetically appeasing environment. You were then triaged by enthusiastic and friendly nursing staff, shortly after seen by a great doctor who laid particular emphasis on the patient-doctor relationship while tasked to perform at international standards. Your pertinent medical records and labs were available to you via an electronic portal such that if you ever were hospitalized or seen at a different location this information would be readily available to guide your medical care. You are also kept within a network of specialist available to you when necessary with access to medical staff for questions and appropriate triage by phone 24/7. Would you not continue to patronize this service? Would you not rave about it and tell the next person? Most importantly, would you personally or allow someone else close to you accept service inferior to this new experienced standard?

Once this grew to a critical mass and consumers as expected flock to the superior experience the market itself would have to adjust. At that point it's all economics driven by you the consumer.

Yes, I am in contact with major stake holders in Nigeria. I understandably can't get into those specifics at this point. Rest assured not only is change possible but it will come because Nigeria can't afford to lag behind forever.

In order to be considered for acceptance into a medical school in the US you typically need to have a bachelors degree and have completed at least 1 year of the following course work:
General biology
Physics with lab
General chemistry with lab
Organic chemistry with lab
Calculus
English
Sociology
Psychology

With a competitive GPA (grade point average) which is typically how quarters/semesters are evaluated in the US. Most schools have a cut off of 3.3 (out of 4) and this is then sub-classified into a general/cumulative GPA vs Science GPA with more emphasis on the latter.

You also have to complete the MCAT with a competitive score (considering international status). This typically has a Physical science, Biological science, Verbal and writing section. I personally find that international applicants do best on the physical and biological sciences and tend to struggle most on the Verbal section. This probably has to do with the structure, sometimes esoteric material covered and style of standardized testing that US students are already used to. I recommend expanding your reading into the social sciences, literature and other non science reading material in order to get comfortable with this type of material

Letters or recommendation Research, volunteer and clinical shadowing experiences are also crucial to enhance your competitiveness. Having a research publication at this stage certainly goes a long way because you literally need to check every possible box to separate yourself from the pack.

With that said, the statistics from admission data particularly for international students is quite intimidating. However, I believe one has to equip themselves which as much realistic information/data as possible in order to accurately appraise and prepare for the task that lies ahead. In 2013, a total of 1088 non US citizens/residents applied with 115 successfully matriculating (10.6%). I have attached a link to a website with a great comprehensive chart with matriculation and cost data for the US and Canadian medical schools that consider international applicants. The average matriculation rate per institution appears to be between 0-2%.

Our health sector is so disjointed, that a complete and total overhaul from bottom to top is required (not only logistics, but also healthcare providers and professionals).
Politicians and the wealthy don't necessarily require primary care, because when they catch a cold, they fly abroad for cough mixture(the only time they require our local hospitals, is when the anti-graft agencies are hunting them).
A lot of investment in Persons,training,hardware and logistics is required.
How do you intend to tackle all this shortfalls?
Appears daunting eh. You certainly have a valid point that our health sector is disjointed and a bottom to top comprehensive overhaul would ideally be an appropriate prescription for success. However, I believe its fair to say that it would not be practical or realistic in present day Nigeria. There are a multitude of reasons for the present state of affairs i.e inadequate governmental spending on healthcare (haven't met the previously hopeful terms of the Abuja declaration from 2001), poor regulatory environment, low barrier to entry leading to an excess of sub-par players in the market, increased and sometimes necessary utilization of medical tourism as you referenced to name a few.

With that said, I firmly believe that the private sector has to take charge in these affairs and start the march in the right direction. The public sector can then be positively influenced and will not have a choice but to follow along. I personally have been fortunate to study and practice medicine in a more mature healthcare market with the appropriate infrastructure and cutting edge medical technology. Though the US has its cost and sustainability concerns, the lessons learned can certainly benefit a young immature healthcare market like Nigeria that is still struggling to provide necessary access to quality care.

I aim to provide the average Nigerian consumer with the quality care he/she deserves by leveraging technology, vertical integration of services and the expertise garnered by myself and colleagues. All while utilizing the large medical human capital already present in Nigeria (as evidenced by our significant representation as excellent and high achieving healthcare practitioners across major developed markets), providing the appropriate training to supplement their fundamental medical training while elevating the standard of care, optimizing the logistics of healthcare delivery and technological integration to weed out excess/redundant services in the healthcare system.

It is a formidable task but I would submit that hardworking Nigerians with their associated economic emergence when looking at key economic and population indicators (not just the short term view) are ready for this healthcare revolution. It will require bright minded passionate individuals like myself to make this a reality!

This is an interesting question and I will use the opportunity to address others who might have a similar issue.

Jaundice in itself is just the physical manifestation of someone with elevated blood levels of bilirubin. Without getting into the technical details, bilirubin is a breakdown product of red blood cells and subsequent by products are responsible for the coloration of of feces and urine. With that said, Jaundice in itself is not a disease but rather a sign of an underlying abnormality. This is a very important distinction to make particularly when it comes to our current Nigerian health ecosystem inclusive of consumers like yourself, pharmacists, local herbalists, clinics, hospitals to name a few. It is more important to determine WHY one is manifesting such signs and symptoms in order to then come up with an appropriate assessment and plan. Trying to find a drug or procedure to cure a sign or symptom is the wrong way to approach things.

The case of [b]Jaundice in the adult [/b]is a perfect case in point as the underlying cause can be quite varied. After a detailed history and physical, a liver function test would need to be obtained to accurately classify the type of Jaundice. Broad categories for further classification of the possible cause could be;

*pre-hepatic (occuring prior to arrival to the liver which is responsible for processing bilirubin) such as: increased break down of red blood cells from genetic causes (sickle cell, thalassemias, G6PD deficiency and other complex disorders to name a few), severe malaria, some infections can be associated with hemolysis etc

*Intra-hepatic causes include; acute or chronic hepatitis (typically viral), drug or autoimmune hepatitis, cirrhosis, cancer/malignancy, Primary biliary cirrhosis, sclerosing cholangitis to name a few. These typically will be associated with other manifestations of liver injury (RUQ pain) or liver failure such as Lower extremity edema, ascites, easy bruising and can even progress to involve kidney failure.

*Post-hepatic causes (coming after the liver-conjugated hyperbilirubinemia): typically are obstructive in nature and usually involves blockage of drainage of properly processed bilirubin from the liver. This includes pancreatitis, pancreatic cancer, cholangiocarcinoma, drug or pregnancy induced cholestasis etc.

The point I hope to drive home here is that each one of the aforementioned conditions involves a unique therapeutic approach. Therefore, [b]appropriate diagnosis [/b]is the most important aspect in this process because that is the only way the right therapy/treatment can be determined. The benefits of medications, procedures and surgeries typically only outweigh the associated risks and adverse effects when it is used appropriately for the right disease process or condition. Otherwise, you often end up doing more harm than good.

So please unbranded13, try to find a good doctor who can talk to you, get a great history and physical exam to confirm and try to better understand the cause of your jaundice. After which, he/she can perform the right diagnostic tests to determine how best to move forward. You are sometimes better served by someone who tells you they don't know and refers you to a more qualified colleague who might than by someone who is quick to prescribe therapies that are not based on the proper diagnostic approach.
