The World Health Organisation (WHO) country representative to Nigeria, Dr David Okello, has disclosed that WHO has prohibited the prescription and use of Artemisinin monotherapies and other less efficacious medicines such as Artesunate, Chloroquine, Amalar, and others in order to delay or prevent the development of drug resistance malaria.

Dr Okello who was represented by Dr Tolu Arowolo, National Professional Officer, Malaria(South-West) of WHO stated this in Lagos on Wednesday at a media roundtable organised by Journalists Against AIDS (JAAIDS) as part of activities to commemorate this year’s World Malaria Day scheduled for April 25.

He explained: “WHO has recommended the ban of the production, importation and prescription of artemisinin monotherapies. It has also launched a global plan to contain or control the spread of artemisinin resistant malaria. There is no place for the use of Chloroquine and Amalar in the treatment of malaria in Nigeria. Artesunate alone cannot treat malaria. Sulphadoxine pyremethamine are no longer recommended in the treatment of malaria.”

artemisin monotherapies would likely hasten the build up of resistance against artemisin by the malaria parasite with the likely consequence of a high population treatment resistance,recrusdence and failure in malaria therapy.This is no news as even international trade in pure artemisin is scrict and regulated,the gold-standard is to combine artemisin with other pharmacologically active agents like lumefantrine,primaquine,amodiaquine etc popularly regarded as Artemisin combination therapies(ACTs).The blanket ban placed on chloroquine is still controversial,I see it more of economic-political scheme,most patients still prefer chloroquine shocked,especially those who feel or are convinced it works best for them(interestingly I belong to this group,I have never used any ACT to treat myself anytime I am down with malaria!) and most importantly,the issue of cost remains a major reason I consider it hasty to ban C/Q in 3rd world countries like Nigeria,for as long as the ACTs remain as expensive as they are currently,it bcomes extremely necessary to keep c/q as alternative as failure to provide this alternative would promote the use or patronage of quacks and herbal practitioners for malaria tx while the platform to educate and maintain health education to poor patients would diminish!

this is odd. . .

firstly WHO cannot ban the use of any drug in a country. That power lies solely on the regulatory agency in that country, NAFDAC in our case.

secondly, amalar and the likes have indeed been shown to be useful in preventing malaria in pregnant women, hence their use as IPT.

The reasons for the banning of artemisinin monotherapies, sulfadoxine pyrimethamine and chloroquine is tied to the build up of resistance of the malaria parasite. But note that sulfadoxine-pyrimethamine 2nd and 3rd trimesters) and chloroquine( All trimesters) are still the recommended medications in pregnancy

The current therapies we have now are being safe guarded against the malaria parasite building resistance to them.
i tell you the truth, if the current approved antimalaria drugs fail again we are in for trouble.

And yes, the WHO can regulate drugs worldwide. NAFDAC for your information collaborates on a high level with the WHO and in some instances answerable to the WHO even if not directly.

The recommended antimalaria drugs are as listed below. but please note that self medication is highly discouraged. You have to be confirmed to having malaria before you take these antimalaria drugs. if not you'll be contributing to the build up of resistance. These drugs are not for prophylaxis ( not for prevention) but for treatment of established malaria case. Pharmacies and the laboratories can test you for malaria.

1. Artemether lumefantrine combination drug
2. Artesunate  amodiaquine combination.
3. Quinine for celebral malaria and even in pregnancy ( consult the pharmacist or the doctor first).

others that can also be used are
1. Artesunate -sulfadoxine combination
2. artesunate- piperaquine combination.

NOTE: DO NOT SELF MEDICATE. CONSULT YOUR PHARMACIST OR DOCTOR BEFORE TAKING ANY MEDICATION.

My pharmacist aunt usually gives me Artesunate and Fansidar. I hope there is nothing wrong with that.

Monotherapy is dangerous with the end result that the malaria parasites would develop resistance to the drugs rendering them ineffective and useless. WHO is worried that if the situation gets to this level the world especially African countries would suffer. There may be no replacement if this should be allowed to happen.

As of today, malaria infection is responsible for the highest morbidity and mortality in children and pregnant women in Sub-Sahara Africa.

Well haven’t read the full text of the WHO report . Banning the use of artemesin monotherapies makes sense because use of artemesins alone as mono therapy will promote resistance to the drug and thus render it useless. But a blanket ban on the use of chloroquineand sulphadoxine pyremethamine does not make sense. To begin with Non falciparun malaria(not very common in Nigeria) is stll quite sensitive to chloroquine and it makes economic sense to use chloroquine for treating such malaria even in Nigeria. Besides chloroquine has other uses apart from treating malaria. For the Sulphadoxine pyremethamine group, they are still recommended for prophylaxis and as such should not have been banned.
@obailala. There is no best brand for treating malaria! even the ACTs still being develoaped. Some combinations are known to cause severe side effects are falling out of favour while new combinations are being researched .ACTs are not for every and anybody as presence of some medical conditions would prohibit the use of some of these combinations.
Please discuss with your doctor or pharmacist if in doubt.

Most likely to be because of resistance, in that case it will only be limited for a couple of years soo when next its re-released the drug will be more effective.
IMO the side effects of chloroquine outweighs its benefits, the illness caused by chloroquione in most is worst than malaria itself.

Nigeria should focus more on improved national health, malaria is only bad in cases where immunity is already compromised.

As for me,the only drug I use for malaria is Artesunate;can any medical person advise on any other drug(s) I can take ,now that it has been banned embarassed.

My sister, Artesunate is not banned o. What is banned is the use of Artesunate ALONE. To use the drug it must be ACT(Artemisinin Combination Therapy) i.e you combine Artesunate with say Fansidar.

Don't mis-inform people. That is not the reason the drugs were banned. If you know nothing about this topic, simply keep quiet and allow people that know contribute meaningfully.

@Bluetooth, i gave out the reasons why the drugs were banned and even rolled out the WHO recommended/ministry of health approved antimalarial drugs. I used the chemical components of the drugs and not their marketing names. the post was deleted, I was banned with my posting rights withdrawn. I had to ask why the ban? i wasn't told why but my posting right was released.
