#jamb admission status(coloured)
#jamb admission letter(institution copy)
#funaab admission status printout
#bio data form
#testimonial/attestation-letter,birth certificate,original o'level(both original and photocopy but arrange d photocopy with other forms and hold d originals separately

Normal procedure has it dat ur HOD,admissions officer,school officer and odas are gonna sign ur forms...so rule out every tot dat all would be signed by one person...hope dis helps sha.. I quote "To be completed in quintuplicate(5 copies).." English.....____TO BE COMPLETED not TO BE SUPPLIED...dat means u av 2 fill 5 copies ..and take to different offices...
