SIMPLIFIED:
The second person was able to get visa even with the first already there, same for the third and then the fourth. If the fourth person filled the three other siblings are in USA and still got visa then I believe you can get too.
I feel you should find out especially from the fourth person if he filled the three others during his VISA application so can know what to do also, the chances of getting visa approved with three siblings already is almost the same with you having four.

There are two different dates listed on the I-20, one is the start of program date and the other is class start date. The start of program date is 1 week before start of classes.

For Emergency Appointment, which of the dates do I count 60 days from? start of program date or class start date ?

Date of your orientation would suffice. So it should be the first date not the date your classes start.
