So, I am thinking of going to a hospital, a government teaching hospital, for contraception or family planning. The thing is, while I'm 21, I still look like someone that's still a teenager, many people say I look very young for my age and are surprised when I tell them my age. So because of this do you think it's advise I take along a copy of my birth certificate in case the nurse or doctor asks my age and doesn't believe me when I tell her I'm 21? I know most hospitals usually don't give contraceptives to minors without a parent or guardian's consent so I don't want them to think I'm a minor. I also don't have a driver's license that's why I'm thinking about taking my birth certificate.
Also what else should I expect at a hospital visit for family planning?

Because I'm having sex and I don't want to get pregnant right now.
21 is old enough for contraception though. It's below 18 that's problematic.

I'm not seeking contraception so I can sleep around. I'm in a long term relationship and I'm monogamous. I'm just seeking to prevent pregnancy.
