If only real life situations were Yes and Nos.

Scenario 1:

I have N16m, I make N1-3m/month, the surgery won't worsen her situation -- I'll pay . . . it takes me 6--10 months to make N15m.

Scenario 2:

I am given N16m to start a business, she needs the surgery at N15m -- I will not pay.

OTOH, you still have to look into her prognosis on palliative care which would sure be cheaper. And consider her level of discomfort . . .

That's what happens in real life scenarios.
