My best guess is, a guy used it and in the middle of the act lost it but decided to go back inside without it, in the process stuffing the thing inside the more, then afterwards he didn't tell the girl what happened, and many girls do not bother checking if a guy came out with condom or not, especial if they have sex with the lights off.

If she doesn't have a single partner, the. She wouldn't know who did that, that's the problem. But I bet you the guy that did it knows 100% but covered the silly act up probably so as not to be held responsible.

I have heard of men intentionally cutting condom before doing it with a lady but I can not understand why a man will decide to leave a condom inside somebody you are having fun with (I can not do it, if it happens I will tell you so that two of us will struggle to remove it immediatly!).
As for your parent, pray they react to your sins immediatly and forget it quickly! If it is me, I will not kill you as a parent, but the memory will remain with me till I die and any time I see you...!!!
