Well considering the fact that he doesn't love her anymore, I'd say he do himself and her a favour by letting her go. If she's a good woman like you opined, she's bound to get another man who'll treat her better. Nobody should be forced to be in a relationship out of pity, it never turns out well

He should talk to her and explain. Being with her just because she donated a kidney, will make the both of them miserable later in life. He should let her know how he feels.
