Pressure?

There was no oxygen in the tank.

Oxygen in such a tank is highly compressed.

The set up cannot contain oxygen.

\

The right setup will involve the use of what is called a VOM (ventilator oxygen machine ) which monitors a patients breathing and vitals to dispense the right mix of oxygen and air into the patients Lungs at the right pressure for every given instance ..That set up in the hospital above was just a cylinder to a valve straight into tiny hoses and maybe into a mask ..That set up can never save a life and I think the hospitals just set them up to decieve gullible people that they ate doing something ...see below a proper oxygen ventilator

I have also had bad experiences with duro soleye. I took my new baby there cos he had jaundice and at a time I had to shout and scream at the nurse and Dr. Kasali. They are so unprofessional. All that the nurses are looking for is to help cut bills to their advantage (you sorting them afterwards). On my exit from the hospital, I tore their card at the reception for everyone to see.

Sorry about your loss.

Actually with a regulator they can still deliver low flow oxygen to a patient. But that connection is so highly compromised that there is a 100% probability that there was no oxygen in the tank. Question: Were they monitoring his SpO2?

The picture you showed is a ventilator. That is a whole new ball game that requires placing an artificial airway (in this case endotracheal tube). The hospital is not equipped to handle such critical care cases.

so sad, may the good lord comfort you. but I have few questions.
why did you leave the situation to become so critical before assessing medical care? for a fourteen year old to be able to walk unassisted means it was a very serious case and it would have been ongoing for a while.
(2) you appeared to be uncomfortable with the quality of care your child was getting and the competence of the care giver. what stopped you from seeking help elsewhere?
if the doctors really did all you said, they are guilty of misconduct, but you are equally culpable. you should take your child somewhere else once you are convinced he\ she is been ill treated.

Here's the deal.

Healthcare is a partnership between patient and doctor.

The mother failed and she failed woefully. All the signs were there but she ignored them. Why did she ignore them? Maybe financial, who knows.

Nigeria is not Russia, she should have left the hospital inside of 5 minutes.

As I plan my retirement back to Nigeria, healthcare is a big elephant in the room for me. In fact, my next trip is solely dedicated on researching adequate healthcare in Ibadan. I know I will have to go back to the states for yearly checkups, unfortunately.

PS: I will not step a foot in UCH.

An engr should actually fix the oxygen cylinder or to even change it. You need spanners and all sorts to do so. It's really not part of the clinical staff's job description to do that. I dont expect a nurse or Dr to be carrying spanners about.
There are reasons we have engrs, handy men in hospitals.
It's a pity that as usual, the management of the hospital is useless.
She can sue the hospital tbh.
I'm wondering though, a child that needs long term oxygen, who's acutely ill, really shouldn't be in a private hospital.

Firstly, RIP to the young boy, Tobi. May you find rest at the bosom of the Lord
Secondly, this is an incomplete story. My question is, what brought Tobi to the hospital?
before a CT scan can be requested on a patient, it must be something serious
However, the doctors should have been ready for the next line of action before the arrival of the CT scan result
especially when the patience's life is threatened.
You spoke well.Same question I asked initially. What was the diagnosis? Why the request for a CT scan?
There must have been something serious the boy was dealing with?

As it is expected, I could pick some holes in the mother's narrative, most importantly despite her inconvenience and knowledge of the fact that her son wasn't getting the best of avail able care her money could pay for, she kept him there with the untenable excuse that he was on oxygen. He could have been moved even with the oxygen mask.
I am not holding the forte for my colleagues, if her account is true, they did really poorly and should face the disciplinary committee but as we all know, there are two sides tons coin.
Sound judgement can only be made after hearing both sides of the story. Most of what I see in the comments are biased and preconceived judgement.
I wish the grieving family fortitude for their loss.

Thanks brother, and if she feels the medical personnel were negligent and didnt offer the best care to her ward she should channel her complaint to the disciplinary committee of the Medical and Dental Council of Nigeria and not the social media....May her sons soul rest in the bosom of the Most High God..
P.S Doctors actually aint supposed to know how to assembly oxygen cylinders, there are handy men for that. Secondly, the information is incomplete, the actual diagnosis wasn't given. For a CT to be ordered then an assessment or suspicion of a cerebral condition was entertained. Also this is an opener for all, patient's have the right to ask questions and demand better care from their doctor's and if not provided, leave that facility to a better one. Moreso, specialized and better cases should be taken to teaching hospitals.
The doctors erred if her account is accurate and could/should be held liable

Generally, I fear for private practice in Nigeria they are gradually losing their Glory. My younger sister almost died at a private hospital somewhere in Lagos here but for the quick intervention of my self I had to demand that the doctor refer her to a general hospital, we were told categorically that if she had spend an extra day she would have died, she had intestinal obstruction and Adhesion and was been treated for God knows what after been operated upon for CS in that same hospital, we had to rush her back when there was so much complain from her,a week after discharge, a week after readmission the doctors were at there wit's end, I kept goggling on her complain, goggle was giving me intestinal obstruction and Adhesion from the cs she did, it never occurred to the doctor to do scan or x-ray to check,I insisted on her been referred, the referral was the grace she had, quick actions saves life, upon this I was mandated to pay if not no referral letter I paid for everything and took her to general hospital, Badagry,as that was the only general with bed space as at that time. I thank God she is alive.

A Grieving Mother { Mrs Stella Offor BAKARE} Is crying For Justice For Her Deceased Child.
Here is Her Account of what actually transpired:
=================================================================

Tobi, my only child (a male, good-natured 14-year-old) was admitted into Duro Soleye Hospital, Allen Avenue, Lagos State on the morning of Saturday, 10 June 2017. He died on the morning of Tuesday, 13 June 2017 due to the negligence of Doctors Korede and Awe , who were on duty and their employers, Duro Soleye Hospital.

What happened:

1. When Tobi was admitted, the hospital, surprisingly, had no wheel chair to take him to his ward. The nurse on duty put him on my back to get to the ward.

2. Duro Soleye Hospital placed Tobi on oxygen on Sunday, 11 June 2017. The oxygen cylinder was plastered and obviously not in good condition (see attached pictures). This had me worried, but I was more concerned about my son's survival.

3. Tobi had a CT scan at Echoscan on Monday, 12 June 2017. Duro Soleye had no functional ambulance to take us to Echoscan for the test, we had to pay N5,000 for the laboratory to bring their ambulance to pick Tobi up.

4. I repeatedly had to call the attention of the nurse or doctor on duty to attend to Tobi, to replace the drip, or to carry out their lawful duties. The nurses on night duty were never prompt or responsive, always complaining and reluctant to do their work. Throughout our stay in the hospital, the bed sheets were never changed. The floor was also cleaned with only water, instead of disinfectant.

5. We got the CT scan results at about 5pm. The doctors [OE2] on duty saw the results and had no clue how to proceed with the treatments. This was clear evidence of the doctors manifesting clear incompetence in the assessment of Tobi. At that point, I began to panic but could not take him to another hospital because he was already on oxygen. The doctors on duty tried in vain to get directions from the Medical Director, Dr. Odunowo, but could not reach him till about 11pm.

6. In the evening of that same day (i.e., Monday, 12 June 2017), I asked the lady doctor on night duty (Dr Korede) and the nurses to change to another oxygen cylinder to avoid midnight exhaustion. I did this, because their practice was to sleep throughout the night. Regardless of my lawful directive as a paying client and mother of the patient, they refused to change it, insisting that there was still a little oxygen left.

7. The oxygen got exhausted while the doctors and nurses were sound asleep. The doctors and nurses had even forgotten to do the routine ward rounds and could not categorically say when the oxygen got exhausted! The doctors and nurses on duty failed to attend promptly to Tobi, even though he required urgent medical attention.

8. Due to my diligence as a mother, I discovered that the oxygen was exhausted, I called the doctor and nurses to change to another cylinder. None of them knew how to fix the new cylinder, they were arguing among themselves whether it was 'rubber type' or 'plier type'! They subsequently called an elderly man, hospital driver, from downstairs[ who fiddled and fiddled with it and seemingly "fixed" it. Prior to my discovery, the doctor and nurses failed to change the oxygen cylinder, a task which should have been done under any circumstance for the good of Tobi.

9. The doctor (Dr Korede), nurses and the elderly man then left the ward and the doctor switched off the light saying light can damage his eyes. This heightened my fear; after some minutes I switched on the light and called out to the doctor, who came and declared Tobi dead. It turned out that no oxygen passed from the cylinder to Tobi!

10. Also, mucus in his mouth was not sucked all through the night. Tobi was unlawfully deprived of his right to life by the actions and inactions of the medical personnel on duty. He died a horrific and painful death with plenty choking mucus (picture attached), which the Medical Director of the hospital, Dr. Odunowo, saw himself.

I informed the Medical Director of this development when he resumed for duty on Tuesday, 13 June 2017. I also wrote a formal letter (copy attached) to the hospital, which they refused to acknowledge. I gave the hospital my phone number.

I want justice for my son, whose life, dreams and aspirations were illegally cut short. One of his dreams was to study Medicine in the university, so he could help save lives. Unfortunately, the same institution he looked forward to being a part of, woefully failed him. Doctors owe their patients a duty of care and must never be negligent in their duties.

I repeat, it's not the responsibility of the clinical staff to fix oxygen cylinders.
Because I change bulb in my house, or do minor repairs on my gen at home like changing the plug or putting fuel in the gen doesn't mean as a Banker/Dr/Nurse/Pharmacist/accountant/lawyer etc i have to do same at work.
It will only be a stupid management or M.D. that would expect his clinical staff to do such. It's simply not part of their job, and because they tried to even do it, doesnt mean they would succeed at it as obviously seen in this case. Because despite all their attempts, they didn't succeed in getting it to work.
If you feel Nigerian Drs know every other field, then I'm sorry nobody can help you. It's really stupid for people to blame Drs and other clinical staff for equipment that aren't working, for the generator not working, for the ambulance not working or being functional, for the trolley being spoilt or not available.
That's the responsibility of the management to ensure the hospital equipments are working, and available. It's also their responsibility to have an engr/ technician on site to do such.

Everyone in any establishment has their job description, usually clearly spelt out before starting work.

Now, to be honest, the Nigerian health system needs a complete overhaul. I'm not sure we can ever get there in the next 50 years with the way things are going
Only paediatric trained nurses should attend to children. Hospitals that don't have paediatricians shouldn't accept children. Though, we really don't have enough Drs and nurses, talk more of specialists but private hospitals need to just suck it up. It may mean having to cough up more money to attract specialists.
Private hospitals shouldn't be involved in managing critically ill patients or even provide ITU care save for a few who have the manpower and the facilities.
I could write pages on this, but I can't shout.

R.I.P Tobi. I hope a thorough investigation is carried out, and the govt also gets involved. If there's any case of negligence, those involved should face the music. If it's the mgt fault, they should also face the music.

Haha, what's your beef with UCH?Most of the best hospitals you will find in Ibadan are owned or partly managed by UCH consultants.

I say UCH collectively which is a combination of hospital building, medical instruments and repair/calibration, direct primary medical staff like Doctors and secondary medical staff like nurses.

Each is an important leg of the stool that is absolutely required for a hospital to work effectively and efficiently. Remove one and proper patient care is missing and the result could be deadly. (Just look at the sham in the picture by OP).

We'll have to define what is considered best. Somalia I think is better than South Sudan but neither is as good as Holland or Italy, an analogy.

Two trips ago I had a crown that came off and I went to some dental facility near Cocoa House to see about affixing it back. One look at the place, the instruments and the total unhygienic environment I left immediately and endured a loose crown (very painful) for two weeks until I got back stateside. I won't even let my Dog be treated there. Are there better dental clinics in Ibadan? Possibly, but this one shouldn't be in existence.

Much of my career in the US has been spent in the hallways of most of the top rated hospitals in the developed world, so I'm a bit jaded.
