Aunty. Someone did page zero, I appreciated it by assimilating the information there and asking only questions that need real life experience of people (exception of my NOC question which I just had to seek opinion of our in-house expert). Based on what I assimilated, I have been able to assist those in the same stage or behind me in the process.

I never criticised those who help, actually gave due accolades. Just said that it's not by force for me to be like them.

After receiving my PPR mail, I sent my passport as well as original marriage cert, sworn affidavit and newspaper publication( my former passport used at the initial stage of the application carried my maiden name)

The passports( mine, spouse and child) was returned but the original docs were not returned.

I ve sent a mail to Accra office but no response yet.

please advise; how do I contact them? Are there chances of the docs being returned?

Calm down, it's only the US Embassy I know that shreds application documents anyhow. Others keep for a certain time limit. Besides, it's human beings like you and I handling the docs no sane person will shred another person's originals. So to start with, your documents aren't gone. Just keep trying to contact them on how they can be returned. Please be humble in your request and expect some shakara because these people usually state in one way or the other that you should never send your original documents.

I thought so too but hospital is saying that I have to come with it. The receptionist seem to be mistaking the process for a requested medical. Please anyone with a copy of the form should please draw my attention so I can send me email address.
The next appointment is currently 23rd of may and today is just 2nd so I am trying to avoid delays once my ECA is out.

I had experience once with the UK Sheffield visa issuing office. Till date, they never returned some original docs I sent in with my visa application. Good luck.
