Don't worry. You don't even need the Bachelor Certificate unless you did engagement ceremony and are worried about the CO thinking you are married. And even a Sworn Affidavit at a High Court can even suffice.

@Glorioussugar, there are two bar code numbers on the Interview Confirmation Printout. One will be that of the DS160 the other will be that of the Interview Confirmation. So as long as you can see a bar code number on the printout that matches with the DS160 number, you are fine.

I think you really need somebody who knows the process to be physically with you so as to guide you properly.

You don't need any appointment. Just go there with the NVC Letter, your international passport, your passport photo (make sure it's the size they want: 4 x 3), your ATM card (for payment) and records of any past immunization you've done in the past.

The barcode is a mixture of letters and figures and it should be ten in number.The one you referring to that has eight numbers is the UID( the one with eight numbers is at the bottom right of the DS 160).

No wahala, the immunization record and medical history is just to guide them on the kind of vaccine to give you. Most of the time they don't even have any regard for the immunization record because they cannot verify its authenticity. They will also ask if you've had chicken pox in the past. If You say yes then it means they won't give you that vaccine.

If it's QLife Lagos, there was no need to book before hand. The requirement is to get there early before a certain time (I think 10am or 11am, can't remember)....with the required documents.
However, i think it's best to call (or visit) to confirm the documents you need for the medical screening.
