D Request of Medical Exam (Required) is underd supportng documents page. N if shd click on d ? There it always bring out indicate ifu wuld lik CIC tosend u instruction n form for a medical examinattion byfillingouta a Request for Medical exam form. After clicking on d form which is in pdf,d form is tittled REQUEST FOR MEDICAL EXAM i scrolled down to where Do u need a medical exam? I clickedon ys, i want to complete a meedicaal exam. THEN to save d file to often tells me SAVE A BLANK COPY OF THIS FORM. Is not allowing me tosave n upload on cic. Pls am sorry to bore u with my stories. PLs help me wot shd i do. Thks to all dt replied n said i shd download n fill dn upload back,pls do u think dt is d right tin. Pls my pple hlp me out am in a dilema.


Also pls i notice somein on th optional doc where u av detaiils,doc name , help and options. After uploadng ddoc on option is often brings out 2 boxes ie update infromation and remove documeent. Is dis right cos on other documents lik supporting documents it doesnt bring outremove document on d option side. Pls wot do ufink.

Application to Change Conditions, Extend my Stay or Remain in Canada as a student (1294E)

The correct form u r to fill is the Application for a Study Permit Made Outside of Canada [IMM 1294)

Then the question :Have you visited or lived in any one of the designated countries for 6 consecutive months in the last year?

U shud answered NO, except uv bn in a foreign country for @ lst consecutive 6months prior to ur application, thats wen u can click YES.

If u filled no, ths are the checklist u shud av

awe2015: How about if one is not sponsored by the parents but only sponsored by his uncle or friend who can only provide business details as stipulated in your response... Is it advisable to include my parent's landed properties? Such as land docs?

Sure that helps a lot.
The fact that ur parents aint ur sponsor doesnt mean u cant include thr documents.
Add ur mum, dad,sister, brother, niece,nephew documents....this all proves u have a gud home tie.

This is a very tricky and confusing question!
This particular question borders on hw long uv bn working. If uv bn working for 3-5years upward, its very logical for u to include ur personal statement of account in ur application.
The vo expects a prsn working for that long to @lst av a savings evn tho u r nt sponsoring ursef.

I guess this is aw to answer that aspect.
So wats ur take?

*school certificates( pry 6 - NYSC)
*international passport
*digital passport
*application form
*family information

Bro sorry for ur refusal.

First of all, u have no home ties of any sort in ur documents . U shud work on dis by providing landed properties of ur parents n proper documentations to the properties.

Also how ur sponsor derives his/her fund is also essential to one's application. Ur mum's employment letter, payslips, tax clearances will b fine. If shes self employed, she need to provide business registration documents of her business, her biz transaction receipts, tax receipts....All this will go a long way in proving ur home ties.

Another documents u left out is ur payslips n study approval letter from ur place. All ths are also essential to ur application.
The study leave approval letter is infact very important as it shows u r nt jz leaving ur work without the necessary approval. It also serve as a very good home tie document.

A very gud letter of explanation shud do the trick on ur purpose of visit.
Write a convincing letter on why u going for ur course,y canada n most importantly wah u intend to achieve with the program @ the end of ur study.

Ths are few points I can help with. Am sure other members of the house will av other points for u.

chrisviral: He did not answer a question wrongly, im very sure i during my application i asnwerred No too, algoritm same thing too and another friend of mine, they did the same thing too...

The only difference is that regarding the question.... Have you resided in a designated country in the last six months? Yes or No, i observed that most folks here actually went with No, whem actually its suppose to be a yes ... I believe thats why.... Might be wrong

The problem he's aving has gt to do with uploading medical form!
This has gt nothing to do whether he has resided somwhr for 6months or not.

The only situation whr I think he will b asked to upload medical form is if he had done any cic medical within the last 12months bt in this case he's a first timer applicant.
So how come he has a section whr a medical form is required?
The only logical reason is that he had answered somfin wrong in the questionaires, thats y hes having that section.


First timer applicant av 8 sections to upload to and ths are :

1) main application form made outside canada
2)family information form
3)acceptance letter
4)sponsor details
5) passport photo
6) passport datapage
7) letter of explanation
schedule 1 form

Another section whr u may b asked to upload is if ur program requires u to work.

Bt other than ths, I dont think a first timer has somfin else to upload.

Av u had any medical done in the last 12months? If no....then thrs somfin u answered wrong in ur questionnaire.
Go back n review ur answers to ur questionaires n must sure u answer it correctly.
Thrs neva a section whr u upload anyfin medical if this is ur first time of applying.
So pls gt it rite b4 submitting ur application.

Hello Everyone, I have always been interested in M.sc in Canada more reason why i had to read with all i have while at the university and i was able to graduate with a CGPA of 4.02/5.00. Then i was introduced to an Agent for Admission and Visa processing. After choosing a university MUN precisely, and a course of study with high hopes and optimistic spirit of landing in Canada by January 2015, it seems my dreams is about being shattered. Reasons being that, my Agent said it is impossible to secure an admission into any Canadian university because of my Transcript.

I transferred from one state University to a private university where i eventually graduated meaning that my transcript is from 200-400 level. He insisted without transcript from my former school no university in Canada would ever grant me admission or better still my chances of being admitted is 20/80 that i have to go get transcript that was used in admitting me from the state university to the private university where i eventually graduated. And i don't think getting the transcript is ever possible the school and the department are so disorganized that i had to use influence of a staff in the state university to get my transcript sent to the university where i eventually graduated then in fact i happen to be the first student ever that would get transcript successfully from my department and unfortunately the staff is no longer in that state university so i am helpless. I have contacted admission offices of the canadian school of my choice school to seek their official opinion but it seems the admission office don't have the clearance to answer my query but all other issues was attended too except this issue of my chances... they simply ignored the question as regards my admission chances

Please i need help because i wouldn't want to waste my money......cos now i want to sideline the agent. Can my 200l-400l transcript plus my B.sc certificate fetch me admission letter? I am so confused right now . Please house help me I am so worried right now.

I think ur agent is wrong. U can apply with ur 3 year school transcript.
I did apply to some universities with my 3yrs degree transcript n I was admitted. I entered unilag as a diploma student, so I started from 200l to 400level for my graduate degree.
Get in touch with ur university of choice personally thru email or by calling them directly n make ur enquiry ursef.
Morever, most university base thr admission on ur last 2yrs in school.

zeongeon: Bro british council is charging me 1,500 naira for all 9 documents I need to notarize excluding ielts which they said I will be paying 10k for so that they can send it to d school while the court with charge 10,000 for the 10 documents so if I use the court I will pay 20,000 for everything 10,000 to british council to send result and 10,000 for notarization but if I use british council I will spend 11,500 which would b 1,500 for notarization and 10,000 for sending of ielts result.

Won fe gba ee nii ooo notarization in a court shudnt b more than 2k @ most.Evn if na 20 documents u won notarize.

Special thanks to slimdel annd chatwitfees(kesh). After 3denials.on wednesday I was sent medical instruction. Tho I dnt get the mail notification slimdel just asked me to check my cic account reluctantly I did. Nd behold It was there. But now in 200 level in a university of ado ekiti due to the un necessary strikes and holiday I got admission into saskatchewan institute of applied science and technology (siast) business information system. Under gradute. 22 months of study. It my brothers statement of account is the one sponcoring me.

If ur bro is ur sponsor then i suggest u provide his source of income by getting his employment letter, payslips, tax receipts,id card,bank soa..if he's self employed provide his business registration documents, his business transactions receipts and any other sources of income for him....such cud include landed properties, shares,fixed assets.

All this will go a long way in proving ur home ties.


For ur purpose of visit, length of study,financial situation......A very good letter of explanation will do u the good.
U have to put in writing a very good statement as to y u want to go to canada,y u picked the course u intended on study, hw u intend to carry out ur studies n most importantly wat u intend on achieving with the course and hw ur course can impact ur country of residence.

U also av to explain y u have so many gap year.

My college ID.
My Bank statement with about $ 40,000 funds available.
Acceptance letter
I paid the tuttion fees of 11,000 dollars
Police clenrance
After showing all these i have got the refusal for Canadian TRV.

The reason which they gave me for the refusal was that

You have not satisfied me that you will leave Canada at the end of your stay as a temporary resident. In reaching this decision, I considered several factors, Including:


Your travel history. (I have never been to any other country.)
Length of proposed stay in canada (which was 22 month.)
Purpose of visit. (Which was To studt)
Your current employment situation. (I am a student.)
Your personal assets and financial status. (I have shown my bank statement with about C$40 000 funds available.)


Please suggest me what should i do ?

2) You sed u submitted ur college Id....does this means u r still in college?whc country is ur college? what level of education is that? What level of education r u goin for? What course r u goin for? (length of proposed study n purpose of visit)
3) Your personal account statement of $40000 How do u come about the fund if u still in college? Was the money transferred to your acct in lump sum? Can u provide transactions receipts aw u gt ur fund (personal assets n financial status) .

Answer all dis questions n we will see wat went wrong with ur application

chidimma1: my dad cannot find his original employment letter but has the photocopy, i used it in the previous application, could it be the reason why they doubted the authenticity of my docs, that was part of there reason.

Have the photocopy notarised!!!
