Over the past few years there has been a rapid increase in the number of people using visual display units (VDUs)/terminals (VDTs) both at home and at work. The introduction of VDUs, whilst having many advantages, has unfortunately also created concern about their use and possible health hazards.
The various health risks that are associated with the use of VDUs and that may affect the eyes are:

Asthenopia/eye strain;
Facial rash/dermatitis;
Epilepsy-photosensitive type;
Radiation.


[center]Asthenopia[/center]
Asthenopia is a very common complaint amongst VDU operators. Some studies have estimated that up to 40 percent of VDU operators suffer daily from asthenopic symptoms. The symptoms of asthenopia may be ocular, visual, systemic or functional. The ocular symptoms are sore, tired, tender, itchy, dry, burning, throbbing or aching eyes. The usual visual symptoms are focusing problems, blurred or double vision and fixation problems.

The systemic symptoms are headaches and tiredness while functional symptoms include behavioral changes.
VDU operators have a task that makes severe visual demands and, hence, it is not surprising that there are complaints of asthenopia. One possible reason for the asthenopia is uncorrected refractive errors. It has been estimated that about 30 percent of the clerical workers have uncorrected or inadequately corrected visual defects. Asthenopia is also frequently caused by convergence and accommodation difficulties. Convergence insufficiency and low fusional reserves have been found to be a major cause of asthenopia among VDU operators.

Copy typists have a very visually demanding task. Reports indicate that they are more likely to complain of asthenopia than operators using only a VDU. It has been found that some copy typists prefer to view the copy from the right rather than the left side because their binocular co-ordination and binocular stability are better in this viewing direction. While reading, a person tends to take a head orientation that will bias the position of the text slightly to the right of the median plane. If, therefore, the text to be copied is placed in any other position, the binocular stability may well be affected. It should also be noted that convergence is held more easily below the horizontal than above. Hence the screen and document holder should be situated below eye level.

It has been observed that the length of time spent by an operator at a VDU affects the incidence of asthenopia. Increasing the time spent on the VDU from 3.5 to 6 hour produced an increase from 9 to 45 percent in the number of operators reporting symptoms. This represents a five-fold increase in the number of people with problems, which were caused by less than twice the amount of exposure. Frequent work breaks are strongly advised to reduce the incidence of asthenopia, especially for operators who have no variation in tasks during the work period.

Factors such as general fatigue, I'll health, the use of certain drugs and a tendency to migraine and photophobia are some of the physiological factors that may contribute to symptoms of eye strain. Psychological factors, such as nervous or anxious personality, level of stress, motivation, and interest in the work may also be associated with eye strain. Although these personal factors may increase the risk of asthenopic symptoms arising, the physical factors of the environment and task are generally the main cause.

Several studies have investigated stress among VDU operators. Stress commonly arises when the person's perception of their own abilities are not matched by the job's demands. This is a frequent complaint reported by VDU operators, who perform repetitive, monotonous tasks. To minimize stress for the VDU operators, the task should therefore be designed to avoid or reduce repetitive elements by introducing variability in the workload, instead of long periods of concentrated work. Ideally, the change from conventional office handling routines to VDU-based systems should provide an opportunity to experience job satisfaction by removing the repetitive, simple tasks.

Many systemic diseases manifest in the eyes and HIV/AIDS is not an exception. HIV degrades the body’s ability to fight infections thereby exposing the body to various pathogens that poses threat to humans.
The part of the eye that is most affecte is the innermost surface called the retina, which is why the effect of HIV/AIDS on the eye is commonly referred to as HIV/AIDS retinopathy.

There are blood vessels at the retinal area of the eyes and in HIV/AIDS retinopathy, these blood vessels leak small bloods thereby giving rise to small heamorrhages. There are also small whitish spots that look like cotton wool (cotton wool exudates) in the retina.
The following are the eye conditions that observed in patients with HIV/AIDS.

Cytomegalovirus Retinitis; The word ‘retinitis’ simply means inflammation of the retina and cytomegalovirus is the virus that causes retinitis in 20-30 percent of people with HIV/AIDS. Cytomegalovirus retinitis occurs in the advanced stage of AIDS. In this condition, the retina swells up and bleeds which might lead to severe vision loss if left untreated. Although cytomegalovirus retinitis cannot be treated but the progression can be managed with antiviral medications if the patient seeks immediate help from a qualified eye doctor.

Retinal Detachment; As mentioned above, the retina is the innermost surface of the eyes that is attached to the interior walls of the eye. In advanced stages of HIV/AIDS, the cytomegalovirus causes the retina to separate (pull out) from its attachment (retinal detachment) at the interior walls of the eye. This causes a very severe eye problem that leads to vision loss if left untreated.
When the retina is detached, surgical procedures need to be performed to put it back in its proper position.

Kaposi Sarcoma; This is simply a rare form of cancer of the eyelids and conjunctiva (a thin translucent membrane that lines the back surface of the eyelids and the front surface of the eyeball except the cornea) that occurs in HIV patients. In the eyelids, it forms deep purple-red nodules while in the conjunctiva, the lesions are bright red which resembles subconjunctival hemorrhage. This type of cancer does not usually cause harm to the eyes and are often treated by eye doctors.

Increased Eye Infections; Because HIV degrades the body immune system, the eye is now exposed to various pathogens and foreign bodies that causes eye infection. Two common examples are herpes zoster ophthalmicus and herpes simplex keratitis. Herpes zoster ophthalmicus is caused by the varicella-zoster virus (VZV) and it has the following symptoms; eyelid skin lesions (rashes), red eyes, watering eye, decreased sensation of the eye and poor vision. Herpes simplex keratitis on the other hand is caused by herpes simplex virus (HSV) and it comes with the following symptoms; punctuate dots on the cornea, branching or amoeboid pattern of sketches on the cornea, red eyes and watering eyes. These infections require immediate treatment by an eye doctor.

A HIV patient should always go for a thorough eye examination in addition to other body examination he or she must undergo by various medical specialists.

The end of cataract surgery is seriously approaching as researchers in United States of America develop a new drug chemical that will basically shrink down and dissolve
cataracts.

Cataract is a condition in which the crystalline lens loses its transparency and becomes opaque. It is the leading cause of human blindness affecting tens off millions of people worldwide. The only treatment option for cataract is surgical removal and subsequent replacement with intraocular lens material. But now, a team comprising of ophthalmologists and scientists has tested a steroid-based eye drop in animals (dogs and rabbits) which was able to dissolve the cataracts.

The new drug is called Lanosterol which is a naturally-occurring steroid. The effectiveness of Lanosterol on cataracts was observed when researchers became aware of two Chinese children with congenital form of cataract which had never affected their parents. They discovered that these children have mutation that halted Lanosterol production but this mutation is completely absence in their parents.

So if the children weren't producing Lanosterol and did get cataracts but their parents were producing Lanosterol and didn't get cataracts. Thus, they proposed that lanosterol might stop the defective crystalline proteins from forming cataracts in the non-congenital form of the disease. The researchers produced lanosterol-based eye drop and tested it in three types of experiments.

First, they observed a decrease in the size of cataracts while working with the human lens in the lab.
They later tested the eye drop on rabbits and noticed a sharp decrease from severe cataracts to mild cataracts or no cataracts at all.
Finally, it was tested on dogs with naturally occurring cataracts and they responded positively to the eye drops with severe cataracts shrinking away to almost nothing.

Wonderful, right? But before you get too exited, remember that the research did not prove whether vision was actually improved in those animals. Only that the crystalline lens transparency was enhanced. But certainly, the research is a step in the right direction.
In addition, if the eye drop is effective in humans, then there is a possibility of using the lanosterol-based drugs to dissolve the clumsy crystallin and aggregates found in the brain of Alzheimer's and Parkinson's patients.

The researchers next step is to discover how Lanosterol-based eye drops are carrying out their shrinking response on the cataract proteins and to progress their research to human trials.
