I know what killed both the mother and child because in 2021 I was also a victim of this kind of incident, but I was lucky only the baby died, at one hospital here in Benin City, Edo state.

The reasons are as follows:
1. Never induce labour due to ruptured membrane, because after the ruptured membrane the baby will adapt to the new environment inside the mother's womb, and original labour will come on its own in 2 to 3 days after the outflow of water from the mother.

2. Avoid overly large babies by shunning any thing cold drink even if it is cold water, and also avoid sweet mouth (too much sugar).

3. Eat light food only, once you notice labour.

4. Always deliver at local birth attendants place, the rate of safe delivery from these traditional midwives are 99.9 percent over herein Benin City Edo state. Their services are amazing.

The woman in charge works with Ubth ( tho retired) but she runs her private delivery services in a rented apartment.
I fought the decision for weeks but bra... If I'll have more kids, they will never smell a regular hospital.
These woman's deliveries ( with God's will tho) are snap of the finger.
My second child was the fastest, immediately my wife made the first attempt to push, I picked up my phone to call my spiritual leader to join me in prayers b4 the call rang out, I heard my baby boy crying in the labour room.
My jaws dropped, we barely spent 10 hrs in that place both arrival and discharging of mother and baby.

Firstly is this the same hospital your first born was delivered?

Not all that glitter is gold.
I have two kids and my wifes deliveries were all "snap of the finger" and the hospital is what you will call physically "quack".my MIL insisted that's the birth place she wanted for her daughter. The matron has a clean track record of successes.
I was skeptical during my first born but the snap process delivery is an experience you won't get from all the big sky scrapper hospitals I wanted my children to be birth in.
Experience matters in child birth and it's not always big hospitals.
