I think you should take it one step at a time. I want to believe you've secured a job. If you haven't you should start working on it. I got a format from NL some months back. Just Google it & include NL while googling. By the way you don't need to tell them. As soon as you inform them about the request letter, they'll then ask you to either go compose it yourself or not worry.

Yeah, I chose Lagos and an Aunt that works with NYSC is helping me out with it. She also asked that I find a place in Lagos where I want to serve so she can help. I chose Ajah, because I have relatives there willing to accommodate me. I would like to be double sure I would be posted there, so I want to go there a week before camp, to get a request letter from a firm or ministry, but I will settle for a school if I don't have one. But I just want to make sure I get posted to Ajah.

I would like to know, If i can enter the Camp with Afro.

The answer to the first question is 'Yes'. No serious organization would accept you without asking for your CV.

To the 2nd question, I don't think you'll be afforded such opportunity. You can only be allowed to if peradventure you were posted there but they later rejected you.

Depending on the course u read, you may not receive more than 15k serving in a ministry. You may however receive a little above that if you get a good secondary school in Ajah. Just an advice!
