On May 29, during an interview session with a select group of journalists to mark the first anniversary of his usurped presidency, Umaru Yar’adua tried to downplay the severity of his health problems. But several medical doctors knowledgeable about his physical condition have reaffirmed to Saharareporters that Yar'adua was diagnosed a few years ago for Churg Strauss disease—and has been on a regimen of treatment since. According to the same sources, Yar'adua is also dogged by asthma, which may have been responsible for his persistent cough and labored breathing throughout the interview session. Yar’adua admitted to having been treated for a kidney condition in 2000. Even so, he tried to create the impression that his symptoms, which he admitted to in the interviews, were unrelated. But the doctors who spoke anonymously to Saharareporters said the symptoms Yar'adua admitted to on national television “are consistent with the Churg Strauss syndrome.”



Dr. Salisu Barau Banye, Yar'adua’s personal physician, has been treating him over the years for asthma. Our sources indicated that, while governor of Katsina, Yar’adua had equipped the General Hospital in Katsina with dialysis machines to take care of his kidney treatments. Dr. Banye acted as the hospital’s medical director at the time. In the carefully packaged, nationally televised live interview, Yar'adua admitted that his arteries had dilated by 21/2 diameters, a condition that our sources describe as “vasculitis,” a major feature of Churg Strauss syndrome. Churg Strauss has a plethora of other symptoms, which may include symptoms such as psychiatric reactions as agitation or restlessness, which manifested by the inflation of the arteries of the brain. The syndrome also includes a condition called angioedema (accumulation of fluid in the face, which can also indicate the accumulation of fluid in the upper airway and compromise breathing in extreme cases), well-known cause of sudden death-a swelling of the face, can affect the ability to breathe, which Yar'adua rightly admitted during the chat.



One of the doctors who spoke to us said that medical ethics respects a patient’s right to confidentiality about his medical condition—but with some exceptions. “A man who is in public office owes it to the people he leads to be candid and upfront about his medical issues. From that perspective, President Yar'adua ought to have disclosed his true condition instead of picking and choosing symptoms that bugged him at different times of the life of the syndrome before it was properly diagnosed.” They allude to recent disclosures made by the two US frontline presidential candidates-Barack Obama and John McCain as well as Fidel Castro, and US Senator Edward Kennedy. Yar'adua, who takes steroids, was flown to Germany in 2007, weeks before the massively rigged April elections that brought him to power. He has since been in Germany twice for medical emergencies, even though one of the visits was disguised as a quasi-official visit to the country.



In April, Yar’adua was flown to Germany after he had a scary relapse. He spent 11 days in Wiesbaden , Germany , close to five of those days in intensive care where he was reportedly intubated (put on respiratory support). He has since been trying to sell the hoax that his ailment was a simple case of allergic drug reaction. According to one of our medical sources, “What he didn’t tell the Nigerian public during his media chat was the underlying reason behind the adverse reaction to anti-malaria medication. Nor did he reveal that he was self-medicating on Metalcafin, an off the counter anti-malaria medicine when in fact he didn't have malaria as he claimed.” Churg Strauss syndrome has no known cure, even though it can be managed to prolong the life of its victim. Even so, the doctors who spoke to Saharareporters insisted that a patient with Churg Strauss was not likely to have the energy to carry out demanding official duties.



Asked to comment on Yar’adua’s efforts to give an impression of improved health, one of the doctors said that a shot or two of carefully controlled substances—steroids--could always boost Yar'adua’s short-term ability to be active and to function at some minimum level of capability. “The steroids in his medicine cabinet could help him somewhat upbeat and healthy, but they certainly can’t sustain him for the long term,” said the doctor. During his presidential campaigns, Yar’adua had responded to lingering concerns about his state of health by bragging about his ability to play rounds of squash games.

First Description
Churg-Strauss Syndrome (CSS) is a systemic vasculitis. This disease was first described in 1951 by Dr. Jacob Churg and Dr. Lotte Strauss as a syndrome consisting of “asthma, eosinophilia [an excessive number of eosinophils in the blood], fever, and accompanying vasculitis of various organ systems”. CSS shares many of the clinical and pathological features of polyarteritis nodosa (“PAN”, another type of vasculitis). Churg and Strauss discovered that the presence of granulomas as well as the abundance of eosinophils distinguished this disease from PAN. Another name for Churg-Strauss Syndrome is Allergic granulomatosis.

Who gets Churg-Strauss Syndrome (the “typical” patient)?
The typical patient with CSS is a middle aged individual with a history of new-onset or newly-worsened asthma. The distribution of the disease among males and females is approximately equal.


Classic symptoms and signs of Churg-Strauss Syndrome
Asthma is one of the cardinal features of CSS. Asthma symptoms may begin long before the onset of vasculitis – e.g., many years before any other symptoms of CSS arise, and long before the diagnosis of CSS is made. Other early symptoms/signs include nasal polyps and allergic rhinitis.

The next phase of the disease is often marked by eosinophilia, the finding of an excessive number of eosinophils in the blood or in tissues. An eosinophil is one subtypes of white blood cell. Normally, eosinophils comprise 5% or less of the total white blood cell count. In CSS, the percentage of eosinophils may reach as high as 60%. In the picture below, the eosinophils are shown by the dark pink stain.



The third phase of the illness is a vasculitis, which involves the skin, lungs, nerves, kidneys, and other organs. Particular mention should be made of the frequent devastating involvement of the nerves (called mononeuritis multiplex), which produces severe tingling, numbess, shooting pains, and severe muscle wasting/power loss in the hands or feet. The list below contains the organs commonly involved by CSS and the specific disease manifestation(s) in each organ.

Vasculitis lesions in heart, can lead to congestive heart failure or a heart attack

Nerve
Peripheral nerve involvement including pain, numbness, or tingling in extremities (neuropathy/mononeuritis multiplex)

What causes Churg-Strauss Syndrome?
The cause of CSS is unknown but is probably multi-factorial. Genetics may play a small role in the disease, but CSS is almost never seen in two members of the same family. Environmental factors such as exposure to industrial solvents may play a role in susceptibility to this disease, but this is largely speculative. Infections may be the inciting event(s), but to date there is no definitive evidence of this.

How is Churg-Strauss Syndrome Diagnosed?
Among all of the vasculitides, asthma is a distinctive feature of CSS alone. However, not all patients with asthma have vasculitis – only a tiny minority do, in fact. It is the specific combination of symptoms and signs, the pattern of organ involvement, and the presence of certain abnormal blood tests (eosinophilia, in particular) that help the doctor make the diagnosis. In addition to a detailed history and physical examination, blood tests, chest X-rays and other types of imaging studies, nerve conduction tests, and tissue biopsies (e.g., of lung, skin, or nerve) may be performed to help diagnose CSS.

The American College of Rheumatology (ACR) has established criteria that must be fulfilled in order to classify a patient as having CSS. These criteria were intended to distinguish CSS from other forms of vasculitis (for the purposes of research studies). Not all patients meet every criterion. Some, in fact, may have only 2 or 3 criteria, yet their physicians are still comfortable classifying their disease as CSS. The ACR selected 6 disease features (criteria) as being those that best distinguished CSS from other vasculitides. In order to be classified as a CSS patient in a research study, therefore, a patient should have at least 4 of the 6 ACR criteria.

These criteria include: 1) asthma; 2) eosinophilia [>10% on differential WBC count]; 3) mononeuropathy; 4) transient pulmonary infiltrates on chest X-rays; 5) paranasal sinus abnormalities; and 6) biopsy containing a blood vessel with extravascular eosinophils.

Treatment and Course of Churg-Strauss Syndrome
CSS usually responds to prednisone. Initially, high doses of oral prednisone are used in an attempt to get the disease into remission as quickly as possibly (e.g., using oral prednisone 40-60 mg/day). After the first month or so, this high dose of prednisone is gradually tapered down over the ensuing months. Other immunosuppressive drugs, such as azathioprine, cellcept, methotrexate, or cyclophosphamide may be used in addition to prednisone. High doses of intravenous steroids (usually methylprednisolone) maybe useful for those patients with severe disease or for those who are unresponsive to the combination of oral prednisone used with other immunosuppressive medications.

Prior to the advent of prednisone, CSS was often a fatal disease. The majority of patients died from rampant, uncontrolled disease. With present therapy, constitutional symptoms begin to resolve quite quickly, with gradual improvement in cardiac and renal function, as well as improvement in the pain that results from peripheral nerve involvement. The course of therapy can last for 1 to 2 years, although the length and type of treatment depend on the severity of disease and the organs involved. The patient’s response to treatment and the continuation of disease control during lowering of the prednisone dose are the primary determinants of how long therapy is continued. Laboratory monitoring of blood tests is very helpful in gauging the activity of disease. Some of the most useful laboratory tests are the erythrocyte sedimentation rate (ESR) and the eosinophil count.

Sorry, if I were in his shoes, I would definitely be spending some quality time with my wives and children.  But trust the "greed" factor in the black skin generally-speaking.

I've always believed that one of the problems with Nigeria is caused by those who out of years of frustration while in Nigeria managed to get out of the country either legally or otherwise. These are people that the new Nigeria don't need.

Thank you. He has time for an interview proclaiming his sydrome but finds it hard to do one good thing that Nigerians can remember him for. If one president will leave office making sure that even only electricity or only corruption or only sufficient food for Nigerian then his/her name will go in the history books. Rather they complain of one ill or another, OBJ was ugliness this Yar'ill is sickness.

All those who claim calling Nigerian politicians out and Nigerians on all whole out on their crap are unpatriotic are the most unpatriotic ones for living in a disillusioned world.

Have you ever been so sick that all you think about is "God when will I be well"? Now, Ya'dua has a terminal illness. Terminal means he won't recover from it neither can it be cured. He has to run into a dialysis unit say 2-3 times a week. D'u know how long a session takes? You'll be shocked. While he may be very concerned about the state of Nigeria he can only be more worried about his health condition. You would have done the same if it were you.

This man needs to be in good health for him to rule Nigeria. Yes, he has to be well enough for him to make proper decisions. Nigeria is too tough a nation to have Ya'dua with his illness as President. Don't get me wrong. I feel very sorry for him but really he didn't have to be the President. Yea may be he got there out of greed and probably thought he could transform an ocean into land in four years. On the other hand, I agree it doesn't take a genius to rule Nigeria but hey a severely ill President won't achieve much either. It's just wise for an unfit President to RESIGN!! Again, he could be greedy embarassed

He's definitely in a better position to correct our problems but also don't forget that a tree doesn't make a forest. The people around him need to buckle up as well and I mean from Aso Rock down to you and I.

Don't say "we". May be you and I want but fact is majority of Nigerians love the corruption. Easy way out is always better but in the end we always see the result. This is the root of our situation and almost everyone has gotten used to that. Until those people crave for a change and are willing to say no to many wrong practices then we'll remain in shambles and keep building churches in every corner of the country wishing change will come.
