It is DEFINITELY NOT an infection.

Infections don't last 10 years.

Due to the blisters, she might sometimes develop a superimposed infection.

But the primary problem that she has had for 10 years is not an infection.

It is impossible for anybody to make a diagnosis online, so you are probably better off taking her to a good hospital.

Hereditary telangiectasia however can cause recurrent AV malformations which may be the reason for the bloody blisters in her mouth.

You can google that and see if her symptoms possibly suggest that diagnosis.

And like somebody else said earlier, doing a swab, microscopy and culture is probably the worst suggestion because the mouth has so many organisms that are normally supposed to be there.

Bottom line, look for a good DOC

I completely agree.
For an illness that has defied therapy over the years, rarer diagnoses should be considered.
Osler Weber Rendu (Hereditary Hemorrhagic Telangiectasia) is on my differential, but the OP has yet to reply the few other questions I asked.
Even the pics show oral telangiectasiae (which I believe is the primary pathology, given the recurrent oral bleeds). Vesicles typically don't bleed when they rupture, so those vesicles aren't so bothersome to me.

Please, please, Gentlemen and Ladies; please help me.

I have a niece who had a mouth problem since she 4 to 5 years old but now she is 15 years old.

The problem is this:

There is some kind of rashes that uses to swell up in her mouth and some are small and some are not and as it is, every night and day her mouth is bleeding due to those rashes on her tongue and inside her mouth.

The thing also affected her teeth and that her teeth look so dirty, she can't eat food that is contained with pepper neighter can she eat hot food nor drink hot things because it pains her.

We have do almost all sorts of medical remedies but to no avail.

I believe that her problem will come to end once I post on this internet of nairaland.com as someone directed me here to ask anything because am tired of google.

Please my brothers and sisters I really need your help on what I should do to make her mouth look fresh, smooth and clean again like every other human being.

See the photos of her mouth, tongue and teeth.
Am thinking it may be a glossal haemangioma....

Hmm... all the diagnosis for just one person. Well since most nairalanders have dropped their diagnosis, it won't be a bad idea if I drop mine.

Since the tongue is d primary site, glossitis will come to mind. The cause may be unknown and moreover she has had it from childhood. 11yrs and on discards all the big diagnosis mentioned here. Cancer, HIV, Stand alone Candidiasis. Etc. Mouth cancer has no burning symptom.

Therefore it could be Benign glossitis with super imposed infection. Bacteria or Fungal. Better still ....Benign Migratory Glossitis.a.k.a Geographic Tongue.

Herpes Simplex Of The Tongue. HSV 1 is also a probable cause. Vesicular eruptions on the mucosal surface mouth...tongue. Raises high index of suspicion.

Since she has burning sensation, can't tolerate spicy or hot foods. Burning Mouth Syndrome a.k.a Glossodynia should also be considered.

Hopefully, a trained specialist in dental care should be consulted.

Am not one.....Am a specialist in Fanny Swindling .

You and Lagusta mirrored my thoughts. The long history and absence of white plaques kinda defeats those. It might be far reaching, but what about amyloidosis?

My oga, the list of differentials is endless....

She needs a team of specialists asap, especially those in internal medicine.....

Since these micro organisms in the mouth are normal flora, the microscopy and culture should be able to detect pathogenic organisms. We can achieve this by comparing the organisms found in a 'healthy' mouth versus hers and use a selective culture media for that 'out of place' organism. My opinion

Please take this girl to The hospital. She has Herpes Simplex Virus. This is the oral version of the virus and it will NOT respond to any antibiotics or antifungal agents. For those saying Oral Candidiasis, it's a huge fail.

Candidiasis is a thrush like coating with sores. This one is vehicular and clustered, very typical of Herpes.

She also needs a HIV test

Waste of time, money and potentially patient's life.

What type of organisms are you hoping to grow from the selective media?

TB, Gonorrhea, Chlamydia, Ricckettssia?

None of those organisms have symptoms like the OP described. They do not infect the tongue.

Some viral infections can last for that long... Remember warts

And someone above asked how she could have been infected with herpes, come on, this is Nigeria, she is a child, she was 4yrs at that time, God knows what she put in her mouth, maybe even a used condom. You don't know what your children do behind you....

But the best bet is for the OP to take the child to a teaching hospital for further evaluation....

Everybody is even screaming UCH as if its the best.... Nigerians and sentiments...

Warts do not cause blood filled blisters.

None of the STDs have symptoms described by the OP.

My oga, I'm trying to say that warts is a viral infection that can last a lifetime, I didn't say this girl has warts.....

And is it because I mentioned condom Habaaaa, that was a joke nah....

I was just trying to say that children can put anything in their mouths; old spoons, rags, used towels, anything....

God bless

You have to go to a hospital and tell them to use a swab stick to collect the fluid coming from the blisters for culture. This will help to identify the specific organism responsible for it and determine its sensitivity. Random antibiotics may not work. Please take this girl for proper treatment. Wishing her speedy recovery.

@Lalasticlala, other opinions are needed please

I bet u r a microbiologist

I could even count up to 50 differentials of that kind of sore. If she is losing weight, I strongly think it is thrush (oral candidiasis), and thus the parents must ascertain their HIV status and that of the child. Oral candidiasis is an AIDS-defining disease.

It could be oral cancer too, since it is involving the teeth and gingivae. Oral erosion from oral cancer can become superimposed with bacteria and other flora in the mouth and cause massive pus formation.

Another is Blastomycosis, then oral Herpes Simplex Virus-1.

She must get tested for HIV first, she and her parents. The sore sample should be tested for culture and sensitivity, and strong antibiotics that the test sample is sensitive to should be given upwards of 21days. It will surely clear that mess of pus and swollen sores first before anything else. Goodluck.
