Thanks. I went to Exams and Records Dept to correct my statement of results. I also applied for a new ID card so as to reflect the new change.
I hope there won't be an issue in terms of transcript as all my documents in school bears Ebere.
NB: I wasn't asked to provide any documents.

I'll advice you to apply for a correction of name on JAMB and submit its indemnity to your school. JAMB would correct the name after the process. The earlier, the better.

***added 07/02/16 - I am so happy that this thread has actually helped out thousands of people...people commenting, answering and helping each other and those viewing in ghost mode We have literally gone from just NYSC camp related experiences to JAMB to all sorts..lol... Gives me goosebumps (Err...maybe not...but..) Thanks to everyone that has helped me out. Couldn't have done it without y'all. Remember, if you need me to address any questions just say my NAME and I'll come running (well...not literally)...and hopefully I'll know or help you find out the answer. Cant answer any questions on JAMB though

4. I’m just hearing about printing out with barcode. I’m not sure I have noticed that on my Jamb admission letter. No I haven’t printed jamb admission with bar code.

Hello..please I have a question that is bothering me..during my jamb registration 2013 the cafe man made a mistake; different state of origin and a different DOB..so the state of origin and DOB on my jamb is different from the one in the insititution.both age is not up to 30..i did jamb regularization and i used the correct details for it.(I don't know if the jamb regularization will correct the data on jamb).i'm done with my HND program...my name is already on senate list and I have done registration..please my question is hope I won't experience any problem in camp and if there is any possible solution,precautions or guide to follow..please help me out.

Gone are the days it was easy to manipulate your date of birth. Any school caught in the act of aiding and abetting age reduction would go in for it.
