When the FH pulsation is absent, the pregnancy is non-viable and will result in miscarriage.

When you experience difficulty visualizing the fetal pole or FH transabdominally, undertake a transvaginal scan. The potential problems of a retroverted uterus and obesity causing poor resolution are immediately overcome and viability can be more easily ascertained once the uterine cavity is found.

Identify the gestation sac, check that it looks regular in outline, and that there is only one. It may appear irregular and collapsing or there may be evidence of hematoma formation. There may be more than one sac so scan the uterus across its full breadth. Measure the diameters in three dimensions and take the average.

If the mean diameter is more than 20mm and there is no evidence of an embryo or yolk sac, the appearances are highly suggestive of an anembryonic pregnancy.

Today I want us to quickly discuss the physiology of reproduction : FEMALE RERODUCTIVE SYSTEM
Anatomy : In the female, the reproductive and urinary systems are entirely separate unlike that of a male. The parts of the female reproductive system include a pair of ovaries, a pair of fallopian tubes, uterus and vagina. These organs are properly and safely housed behind the pubic bone in the bony lower pelvis.

Ovaries: Are the primary sex organs in the female. These are located on either side of the uterus. They vary considerably in size among women but usually measure between 3 and 5cm in length, 1. 5cm in width and 1 to 1. 5cm in thickness.

They are suspended in round ligament, which anchors them to the uterus. Ovaries are the counterparts to the testes in the male in that they secrete the sex hormones....... estrogen, progesterone and testosterone.

At birth each ovary contains about 400, 000 primordial egg cells in primordial follicles. The egg cells have a large nucleus with a clear surrounding cytoplasm that is further surrounded by theca and granulosa cells that secret the follicular fluid.

At the time of puberty around 30, 000 primordial follicles remain, which will either mature into eggs or disintegrate in the period between puberty and menopause.
