You cannot diagnose it so quickly.
Does the blood come with diarrhoea, that maybe sometimes has mucus?

Has he undergone colonoscopy/sigmoidoscopy?

Has he been tested for anaemia/blood test? Perhaps looking pale/tired on examination?

Is he using NSAIDs (Aspirin/Ibuprofen, etc)coincidentally when they occur?
Is anybody else in his family affected?

He needs to do a proper medical work-up to diagnose.

Otherwise the causes of flare-ups (episodes of symptoms if indeed ulcerative colitis) can be missed, therefore not knowing to avoid them. Even something as simple as stress as a cause.

It might just be piles but he should see a gastro-enterologist (digestive tract physician).

Anyway... The history you gave isn't complete hence will be difficult to make a diagnosis... But with that age and the symptoms it is possible he has inflammatory bowel disease like ulcerative colitis (remitting-relapsing type). He needs to see a gastroenterologist (Not all this quacks in most one room "hospital" that claim to be everything). He will likely have a colonoscopy done... All the best

It's neither Cancer nor Ulcer. This is as a result of constant constipation in the past. Your cousin must have ate foods that didn't digest well in the past, frequent stools made his anal walls stretched, as a result of this he developed hemorrhoids (either internal or external).
